摘要
目的 评价动态增强MRI鉴别良恶性肌肉骨骼 (简称肌骨 )肿瘤的价值。方法 前瞻性量化分析 3 2例肌骨肿瘤患者的动态增强磁共振图像 ,采用差别分析法确定良恶性患者的动态增强MRI检查指标有无差别 ,并通过绘制接受者操作特征 (ROC)曲线得出最佳评价指标。采用相关分析法确定定量指标与肿瘤组织血管化程度及DNA指数 (DI)的关系。结果 恶性肌骨肿瘤早期增强斜率值 (slope)的中位数为 1 12 13 % /秒 (范围 0 2 4 5 4 % /秒~ 2 5 4 74 % /秒 ) ,良性为 0 3 0 81% /秒 (范围 0 0 3 0 9% /秒~ 0 882 0 % /秒 ) ,两组间差异具有显著性意义 (P <0 0 1)。slope对肿瘤潜在恶性评估的敏感性、特异性和准确性分别为 87 5 0 %、75 0 0 %、81 2 5 %。恶性肿瘤向心性增强率 (DER)的中位数为 0 110 1(范围 - 0 3 42 9~ 1 0 0 0 0 ) ,良性者为 0 0 92 4 (范围 - 0 5 0 3 9~ 0 5 114 ) ,两组间差异无显著性意义 (P >0 0 5 )。slope与肿瘤组织血管化程度呈线性正相关 (r=0 70 64 ,P <0 0 1) ;与DI呈等级正相关 (rs=0 5 5 3 8,P <0 0 1)。结论 动态增强MRI早期增强斜率值不仅可以反映肿瘤组织的血管化与灌注 ,也在一定程度上反映了其DNA含量的不同 ,为肌骨肿瘤的性质鉴别提供了一种有价值的客观定量?
Objective To study the different diagnosis in musculoskeletal tumors with dynamic MR imaging Methods A prospective study was undertaken in 32 patients with musculoskeletal tumors using dynamic MR imaging Discriminating analysis was used to determine whether the characteristics of musculoskeletal tumors could be distinguished by quantitative indicator of dynamic MR imaging Using ROC curve plotting Correlation analysis was used to determine the relations between the quantitative indicator of dynamic MR imaging and the degree of vascularization, the DNA indexes (DI) Results There was a statistical significance between malignant and benign tumors in the early slope value of enhancement The slope values can give information about the malignant potential of a tumor with a certain degree of overlap between benign and malignant tumors (sensitivity, 87 50%, specificity, 75 00%, accuracy, 81 25%) The difference of the rim to center differential enhancement ratio (DER) was not statistically significant between malignant tumors and benign ones There was a linear positive correlation between the slope and the degree of vascularization ( r =0 7064, P <0 01); there was a rank positive correlation between the slope and DI( r s =0 5538, P <0 01) Conclusions The results suggested that the slope values not only depict tissue vascularization and perfusion, but also demonstrate the differences of nuclear DNA content in musculoskeletal tumors The slope values provide a diagnostically useful quantitative indicator to differentiate benign from malignant musculoskeletal tumors
