摘要
目的观察血液净化治疗(血液灌流HP串联血液透析+HD)海洛因依赖者的心理应激反应的特殊性,探讨护理干预应用于其中的实施效果。方法观察自愿接受血液净化(血液灌流串联血液透析)戒毒者27例,采取认知干预、心理干预、行为矫正、家庭与社区干预等各种护理干预措施,并采用症状自评量表(SCL-90)、焦虑自评量表(SAS)、抑郁自评量表(SDS)以及阿片类药物戒断综合总量化表(OWS)进行效果评定,同时监测其治疗时每30分钟的心率、血压、血糖、去甲肾上腺素、多巴胺浓度,以及血液净化时的有效血流量。结果患者在治疗前及治疗后的SCL-90、SAS、OWS评定分值,以及心率、血压、治疗过程中的血流量对比差异有统计学意义(P<0.05),但血糖、去甲肾上腺素、多巴胺浓度以及SDS的分值对比无显著性差异(P>0.05)。结论综合护理干预能提高患者的心理应激反应能力,改善海洛因依赖者的不良情绪和心理障碍,提高其心理健康水平,对预防复吸和保持操守有一定的促进作用。
Objective To observe the specialty of psychological stress reaction of heroin addicts treated with blood purification（HP＋HD） and explore the effect of nursing intervention on them.Methods Observation has been made on 27 drug abusers who are under detoxification and willing to accept blood purification therapy,which is Blood Perfusion Series Dialysis.And various nursing intervention has been adopted such as cognitive intervention,psychological intervention,behavior modification,family and community intervention.Effect evaluation has also been made through SCL-90,SAS,SDS and OWS.Meanwhile,to monitor the heart rate,blood pressure,blood glucose,norepinephrine,and dopamine concentration in the course of treatment,and supervise the effective time for blood flow during blood purification.Results There is statistically significant difference before and after the treatment based on the evaluation scores of SCL-90,SAS and OWS,heart rate,blood pressure and blood flow in the course of the treatment（P 0.05）,but no significant differences in blood glucose,norepinephrine,dopamine concentration and SDS scores（P 0.05）.Conclusion Integrated care intervention can improve heroin addicts＇ psychological stress reaction capacity,their negative emotions,psychological barriers and their mental health,which plays great roles in the prevention of the heroin addicts from relapse and maintenance on the integrity.
