摘要
目的研究乳腺肿瘤实时超声造影的增强方式及其诊断价值。方法对45例乳腺肿块进行常规超声及超声造影检查,将超声造影增强方式分为周围片状型、周围点条状型、整体均匀型及整体不均匀型,并统计有无充盈缺损。结果乳腺肿瘤26例为恶性,19例为良性。40例超声造影增强,5例超声造影无增强。乳腺肿瘤恶性组周围片状增强、整体不均匀增强的比例高于良性组,恶性组周围点条状增强、整体均匀增强的比例低于良性组,差异均有统计学意义(P<0.05)。以周围片状型增强方式作为超声造影恶性指标,周围点条状型、整体均匀型增强方式作为良性指标,常规超声诊断乳腺癌敏感性为88%,特异性为71%,常规超声结合超声造影诊断乳腺癌敏感性为92%,特异性为86%。结论超声造影良恶性增强模式不同,恶性以周围片状型为主,良性以周围点条状型、整体均匀型为主,有助于乳腺良恶性肿瘤的鉴别诊断。
Objective To investigate the enhancement patterns and diagnostic value of real time contrast - enhanced uhrasound (CEUS) in breast tumors. Methods Forty - five cases of breast tumors were examined by conventional color Doppler uhrasound and CEUS. The enhancement patterns of contrast- enhanced ultrasound was divided into, peripheral flake enhancement type, peripheral spot - band type, integrity uniformity type, integrity ununiformity type. Results There were 26 cases of ,nalignant lesions and 19 cases of benign lesions. 40 cases with enhanceinent, and 5 cases without enhancement. The ratio of peripheral flake type and integrity ununiformity type in malignant group were higher than those in benign group, while the ratio of peripheral spot -band type and integrity uniformity type in malignant group were lower than those in benign group,there was significant difference between two groups ( P 〈 0.05 ). The enhancement pattern of peripheral flake type was used as malignant index of contrast- enhanced uhrasound, while the peripheral spot- band enhancement type and integrity unifnrmity type were used as benign index of contrast - enhanced uhrasound. The sensitivity and specificity of conventional uhrasound in diagnosis of breast cancer was 88% and 71%, respectively, anti the sensitivity and specificity of conventional ultrasound combined with CEUS was 92% and 86% , respectively. Conclusion The enhancement pattern of CEUS in diagnosis of breast lesions was different, most of malignant lesions were peripheral flake type, and most of benign lesions were peripheral spot - baud enhancement type and integrity uniformity type, it is helpful in differential diagnosis of benign and malignant breast tumors.
