经大脑前纵裂入路对前交通动脉瘤夹闭术的发展应用及展望 被引量:2

The Development,Application and Prospects of the Anterior Communicating Artery Aneurysms Surgery Using the Anterior Interhemispheric Approach

摘要 前交通动脉瘤是颅内高发动脉瘤,周围组织解剖结构较复杂,手术难度较大,相应发展了不同的手术入路,如传统的Yasargil的翼点入路、前纵裂入路、改良的半球间入路以及颅底入路等。其中经前纵裂入路早期因为术后并发症多而受到限制,但是随着显微外科的开展和手术器械的改进,经前纵裂入路行大脑前动脉及前交通动脉动脉瘤的夹闭被认为是安全的,行之有效的手术方法。本文就其经大脑前纵裂入路夹闭术的发展应用、技术的优缺点及对其展望等进行简单综述。 The anterior communicating artery aneurysms has a high incidence in intracalvarium, the surrounding anatomical structure is more complex, the clipping of the anterior communicating artery aneurysms has been considered to be relatively difficult, the corresponding development of the surgical approaches is different, such as the traditional Yasargil pterional approach, the anterior interhemispheric approach, the improved interhemispheric approach, and the skull base approach etc. Among them the anterior interhemispheric approach is limited because of its postoperation complications, but with carrying out microsurgery and the improvements of surgical instruments, it is considered to be a safe and effective surgical method. In this paper we reviewed its development and application of technique, technology advantages and disadvantages and its prospects.
