摘要
目的探讨宫颈环形电切术（loop electrosurgical excision procedure，LEEP）联合α-干扰素栓在治疗宫颈上皮内瘤变（cervical intraepithelial neoplasia，CIN）中的作用。方法选择我院CINⅠ～Ⅲ级的83例，随机分为两组。研究组43例，在LEEP术后第1次月经干净后即开始阴道后穹隆放置“一干扰素栓，隔131次，1次1枚，连用3个月，经期停用。对照组40例单纯行LEEP。两组分别于治疗前行宫颈薄层液基细胞学检查（TCT）、阴道镜下定位多点活检及人乳头瘤病毒（HPV）检测，治疗后3、6、9、12个月行TCT及阴道镜下定位多点活检，且术后6、12个月时加行HPV检测。结果两纽手术时间、创面愈合时间、术后并发症，治疗前TCT、阴道镜下活检、HPV检测结果比较差异均无统计学意义（P〉0．05）。治疗后两组比较：①TCT：研究组术后3、6、9、12个月TCT正常率高于对照组；非典型鳞状上皮细胞内瘤变术后发生率低于对照组，两组比较差异有统讨学意义（P〈0．05）；②阴道镜下活检：两组术后3、6、9、12个月CIN均为阴性，差异无统计学意义（P〉0．05）；③HPV检测：研究组术后6、12个月HPV阴性率高于对照组，差异有统计学意义（P〈0．05）。结论LEEP伍用oL一干扰素栓治疗CIN有协同作用，CIN的一次性治愈率明显提高。
Objective To observe the effects of cervical loop electrosurgical excision procedure（LEEP） combined with interferon α-suppository in treatment of cervical intraepithelial neoplasia（CIN）. Methods 83 cases of CIN Ⅰ - Ⅲ in our hospital were randomly divided into two groups. Based on LEEP, The study group （43 cases） was inserted an interferon α-suppository in vaginal fomix after the first menstrual ended（ every other day for once, continued for 3 months） and was disabled in the period of menstrual. The control group （ 40 cases） simply received LEEP without any special treatment. TCT, mutiple punch biopsy by colposeope and HPV test were given to both groups before treatment. After treatment TCT and multi-point positioning colposeopy biopsy were tested in 3, 6, 9, 12 months and HPV testing postoperative was detected 6 and 12 months after surgery. Results The difference of operation time,healing time of the raw surface, postoperative complications, and the results of TCT, colposcopic biopsy and HPV test in the two groups were not statistically significant before treatment （P 〉 0. 05）. The results after treatment： （1）TCT： the normal rate in study group was higher than that in control group in both groups 3, 6, 9 and 12 months after surgery, and atypical squamous cells changing rates were lower than that in control group 3 and 6 months after surgery, and the difference was statistically significant （P 〈 0.05）. （2）Colposcopic biopsy： the results showed CIN were all negative in both groups 3,6,9 and 12 months post surgery, and the difference between the two groups was not statistically significant （P 〉 0. 05）. （3）HPV test： the positive rate of HPV in study group were higher than that in control group 6 and 12 months after surgery and the difference was statistically significant （P 〈 0. 05）. Conclusion LEEP combined with interferon or-suppository has a synergistic treatment effect on CIN, and can significantly improve the cure rate.
