摘要
目的观察重组人碱性成纤维细胞生长因子(rhbFGF)和重组人胰岛素样生长因子-Ⅰ(rhIGF-Ⅰ)与Bio-Oss骨粉复合修复Ⅱ度根分叉牙周组织缺损的效果。方法 2009年4月至2010年1月于中国医科大学盛京医院实验动物中心选取犬4只,人工制备犬上下颌第2、3、4前磨牙的慢性Ⅱ度根分叉病损,共48颗患牙,分为5组,包括3个实验组(A~C组,12颗/组)和2个对照组(D~E组,6颗/组)。A组:Bio-Oss+rhbFGF+rhIGF-Ⅰ;B组:Bio-Oss+rhbFGF;C组:Bio-Oss+rhIGF-Ⅰ;D组:单纯引导组织再生(GTR)术;E组:翻瓣术。分别于植入术前及植入后12周记录颊侧根分叉部位的牙周探针深度(PD)和附着水平(AL)并拍摄X线片,12周后观察牙周组织再生情况。结果多数牙根分叉处PD和AL有不同程度的改善。X线片示A、B、C组牙槽骨几乎充满前磨牙的根分叉,甚至可见较清晰的骨硬板。各实验组均较对照组有更多新生牙槽骨。组织学测量结果可见各实验组新生的牙槽骨高度、牙骨质及结缔组织附着指标均高于对照组;但各实验组间比较,差异均无统计学意义(P>0.05)。结论两种生长因子联合应用与单独应用对促进牙周组织再生无明显差异,但均较单纯GTR术或翻瓣术效果好。
Objective To observe the effect of recombinant human basic fibroblast growth factor and recombinant human insulin-like growth factor-Ⅰcombined with Bio-Oss on the repair of classⅡfurcation defects. Methods From April 2009 to January 2010 four adult male dogs were included in the study. ClassⅡfurcation defects were created at the second,third and fourth premolars. The models were divided into five groups,including three experimental groups,and two control groups. Three experimental groups were Bio-Oss + rhbFGF + rhIGF-Ⅰ,Bio-Oss + rhbFGF and Bio-Oss + rhIGF-Ⅰ,control group with guided tissue regeneration and blank control group with flap surgery. The periodontal depth(PD)and clinical attachment level(AL)at the buccal furcation defects were examined before and 12 weeks after the implantation. X-rays were also obtained at the same time points. Results PD and AL were decreased in most sites. The reduction in groups A was the most significant,and then in turn were in groups B,C,D and E. Groups A,B,C showed alveolar bones were almost full of furcation area,and the relatively clear lamina dura was also found. There were various amounts of newly formed alveolar bone,cement and connective tissues in the dental classⅡfurcation defects in all three groups. There was significant difference in alveolar osteogenesis between the control group and the 3 experimental group. There was no significant difference among the three experimental groups(P 0.05). Conclusion Application of rhbFGF and rhIGF-Ⅰcombined with Bio-Oss compounds is beneficial to the regeneration of the alveolar bone and can avoid the side effects,such as root resorption and bone malunion. There is no significant difference between Bio-Oss + rhbFGF +(or /and) rhIGF-Ⅰ.
