摘要
目的探讨二维时间飞跃法静脉成像(2D-TOF-MRV)与三维对比剂增强血管成像(3D-CE-MRA)在家兔颈部静脉成像中各自的优缺点。方法选取健康成年家兔24只,随机分成A、B两组(各12只),A组行单侧颈总静脉结扎,B组行双侧颈总静脉结扎。分别用两种成像方法于结扎前和结扎后的不同时相行颈部静脉成像,测量椎静脉直径,观察颈静脉结扎端的显示情况,并将椎静脉直径变化情况与兔神经功能评分做相关分析。结果结扎兔颈静脉前,椎静脉直径CE-MRA测量值较TOF-MRV测量值大,两者间差异有统计学意义。结扎兔颈静脉后不同时相椎静脉直径TOF-MRV测量值有恒定的变化规律,呈抛物线型分布,与脑血流灌注变化一致;而CE-MRA测量值的变化缺乏规律性。选择结扎兔颈静脉后6h椎静脉直径TOF-MRV测量值和对照的差值与神经功能评分做相关分析,显示两者呈负相关,但相关系数经统计学检验无统计学意义。对颈静脉结扎端的显示,CE-MRA明显优于TOF-MRV。结论 TOF-MRV与CE-MRA各有优缺点,两者结合应用能提供更多有价值的信息。当颈静脉回流受阻时,椎静脉直径的测量有助于了解颅内静脉回流储备情况,其与脑功能损害的相关关系有待于做更深入细致的研究。
Objective To study the benefits and pitfalls of 2D-TOF-MRV and 3D-CE-MRA in rabbit cervical venography.Methods Twenty-four healthy adult rabbits were randomly divided into 2 groups A and B(12 rabbits for each) .Unilateral ligation of the common jugular vein was performed in group A,and bilateral ligation in group B.Cervical venography with the 2 methods was performed before the ligation and at the different time points post-ligation.The diameter of the vertebral vein was measured in each venography.The recognition of the obstructed point was trying to make.The correlation between changes of the vertebral venous diameter and the neural deficits was studied.Results The vertebral venous diameter measured with CE-MRA was larger than that with TOF-MRV before ligation of the jugular vein.The difference was statistically significant.The vertebral venous diameter measured with TOF-MRV was changed uniformly in the time phases set post-ligation,which went in a parabola-shaped route,the same with that of the cerebral blood perfusion changes.But the diameter measured with CE-MRA was changed irregularly.The cross-correlation was made between the neural deficits and the differences of the diameter compaired at the 6-hour point post-ligation with TOF-MRV,which showed an inverse correlation but with no statistical significance.As for identification of the obstructed points,CE-MRA was much better than TOF-MRV.Conclusions BothTOF-MRV and CE-MRA have their own characteristics,including benefits and pitfalls.Their combining application may afford us more valuable informations.Under circumstances of an obstructed jugular vein,the measurement of vertebral venous diameter may be helpful in the assessment of cranial venous drainage reserve.The latter may play a role in protect brain from congestive damage,and in which further work need be done.
