摘要
目的:总结国内外乳腺癌前病变的导管内增生性病变和小叶瘤病变的形态学特点、分子生物学特征及其与乳腺癌的关系。方法:应用PubMed期刊数据库检索系统,以"乳腺癌前病变、乳腺导管内增生性病变、小叶瘤变和乳腺癌"等为关键词,检索1978-06—2009-10的相关文献175篇。纳入标准:1)导管内增生性病变形态学特点、分子生物学特征;2)小叶瘤变的形态学特点、分子生物学特征;3)导管内增生性病变和小叶瘤变与乳腺癌的关系。根据纳入标准符合分析的文献25篇。结果:乳腺癌癌前病变涵盖了从普通型导管增生、非典型导管增生和导管原位癌、非典型小叶增生和小叶原位癌病理类型的病变,从形态学、分子生物学及临床特点都非常的多变,综合病理形态学、分子病理学、乳腺癌家族史、年龄和辅助检查等因素来评定其乳腺癌风险,行其个体化治疗。结论:乳腺癌是一个全身性疾病,其病因尚未完全明了,有些上皮内瘤变是能在其基因变化、演进的早期就已经转移,而不是在当肿瘤细胞演进到非常高的水平、侵袭性克隆出现时才转移,所以对于乳腺癌癌前病变的跟踪随访及进一步分子生物研究就显得非常重要。
OBJECTIVE：To sum up the domestic and abroad articles related to the morphology and molecular biology of intraductal proliferative lesions and lobular neoplasia as breast precancerous lesion and the relation between them and breast cancer.METHODS：＂Breast precancerous lesions,intraductal proliferative lesions,lobular neoplasia and breast cancer＂ were searched as key words by PubMed from 06-1978 to 10-2009.Totally 175 papers were obtained.Choice criteria：1） the molecular biology and morphology of intraductal proliferative lesions; 2） the molecular biology and morphology of lobular neoplasia; 3） the relations among intraductal proliferative lesions,lobular neoplasia and breast cancer.By extraction according to the choice criteria,among roughing all the articles searched in PubMed,25 papers were finally analyzed.RESULTS：Breast precancerous lesions cover a wide range of different pathologic types such as atypical lobular hyperplasia,lobular carcinoma in situ,usual ductal hyperplasia,atypical ductal hyperplasia and ductal carcinoma in situ,with diversity in its morphology,molecular genetics,biology,and clinical outcome.They should have individual therapies which depend on its pathologic morphology,molecular pathology,family history of breast cancer,age and auxiliary examination to assess its risk of breast cancer.CONCLUSIONS：Breast cancer is a systemic disease of women,and its etiology is not definite.Some intraepithelial neoplasia of breast can be metastasis which is early involved in the pathway of breast carcinogenesis.So the follow-up and more research on breast precancerous lesions are very important to elucidate breast carcinogenesis.
