摘要
目的建立一种简易的甲状腺摄锝率测定法。方法对389例对象行甲状腺显像,计算指标:甲状腺摄锝率(PTUR)、甲状腺大小(ST)、摄取比(UR)、UR×ST、校正比(RR)。以PTUR为因变量,分别以UR,UR×ST,RR,UR、ST,UR、ST、UR×ST,UR、ST、RR为自变量建立六种线性模型A、B、…、F,行线性回归分析,比较模型拟合优度,比较甲状腺摄锝率推算值诊断符合率、负值比率。结果①各模型均方误差依次为18.87、1.45、1.39、10.05、1.36、1.36。②各模型甲状腺摄锝率推算值诊断符合率分别为82.5%、93.6%、94.3%、82.5%、94.6%、94.6%;模型B、C、E、F推算值符合率差异无统计学意义(χ2=0.96,P=0.97);模型C符合率显著高于模型A,D(χ2=28.95,P<0.0001)。③模型A、B、D、E、F的甲状腺摄锝率推算值在摄锝减低或正常情况下可产生一定比率负值,模型C推算值无负值产生。结论以校正比为自变量建立线性模型,具有高拟合优度、高诊断符合率、推算值无负值出现的优点,能准确计算甲状腺摄锝率。
Objective To establish a simple and easy method to measure the 99mTc-pertechnetate thyroid uptake rate. Methods Thyroid imaging was performed in 389 patients. The indexes including thyroid uptake rate, thyroid size (ST), uptake ratio (UR), UR×ST, and rectified ratio (RR) were calculated. Six linear models A, B,…, F were created with the dependent variable uptake rate and the independent variable that was one of the indexes or their combinations: UR, UR×ST, RR, "UR, ST", "UR, ST, UR×ST", "UR, ST,RR". Linear regression analysis was performed. Goodness of fit, coincidence rate and negative value rate of the uptake rate prediction value for all models were compared. Results ①Mean square errors of the above mentioned models were 18.87, 1.45, 1.39, 10.05, 1.36, and 1.36, respectively. The diagnostic coincidence rates of uptake rate prediction value in each model were 82.5%, 93.6%, 94.3%, 82.5%, 94.6%, and 94.6%, respectively. There were no significant differences among the coincidence rates of models B, C, E, F (χ2 = 0.96, P = 0.97), but the coincidence rate of model C was significantly higher than that of model A and D (χ2 = 28.95, P0.000 1). ③The predictive value of uptake rate could be a negative value in the lower or normal uptake rate for model A, B, D, E, F,but not for model C. Conclusion The created linear model with independent variable RR possesses advantages such as high goodness of fit, high diagnostic coincidence rate and prediction value without negative number, and can be used to calculate the 99mTc-pertechnetate thyroid uptake rate accurately.
