摘要
目的用单光子发射计算机断层显像技术(SPECT)及临床神经功能评价的方法探讨通心络胶囊对急性脑梗死短期内局部脑血流量(rCBF)影响及临床疗效。方法选择初发急性脑梗死患者60例,随机分为基础用药对照组(对照组)和通心络治疗组(治疗组)。两组患者治疗前后均行SPECT脑灌注显像和临床神经功能评价(欧洲脑卒中评分及Barthel指数),观察治疗前后患者rCBF(用RI值进行分析)及临床神经功能缺损症状的改变。结果治疗组治疗后RI值明显高于治疗前(P<0.01),对照组则无明显改变。治疗后治疗组RI值、欧洲脑卒中评分(ESS)及Barthel指数显著高于对照组(P<0.05或P<0.01)。结论通心络胶囊能够有效地改善急性脑梗死患者的局部脑血流量,减轻急性脑梗死患者神经功能缺损症状,降低残疾水平,提高患者的长期生活质量。
Objective To investigate the influence of Tongxinluo capsule on regional cerebral blood flow（rCBF） with 99mTc-ECD single photon emission computed tomography（SPECT）brain perfusion imaging,and observe the therapeutic effect of Tongxinluo capsule on acute cerebral infarction.Methods Sixty patients（pts） with acute cerebral infarction were enrolled and divided into two groups randomly：Elemental drugs treatment group（ET group,n=31） and Tongxinluo treatment group（TT group,n=29）.The 99mTc-ECD SPECT brain perfusion imaging,the scores of nervous deficiency and Barthel index were performed before and after drug therapy and the rCBF changes were compared and analyzed with visual and semi-quantitative methods.Results After treatment,there was significant difference in rCBF between ET group and TT group,the rCBF in TT group was higher than that in ET group.In TT group,posttreatment rCBF was higher than the pretreatment level（t=5.175,P0.01）.There were marked differences in the scores of nervous functional deficiency and Barthel index pretreatment and posttreatment in TT group（P0.05 or P0.01）.Conclusion Tonxinluo capsule could improve the rCBF effectively,lessen the neurological symptom,decrease the level of deformity,and enhance the long-term quality of life in pts with acute cerebral infarction.
