联合透析技术对老年血液透析患者并发心力衰竭的疗效观察

摘要 目的:探讨对并发充血性心力衰竭的老年血液透析患者采用联合透析技术治疗的效果。方法:选择2004年1月-2009年1月在本院肾内科血液净化中心长期进行血液透析同时并发充血性心力衰竭的老年患者(n=38),通过自身对照、比较联合血液透析滤过(hemodiafiltration,HDF)、血液灌流(hemoperfusion,HP)联合低温高-低钠序贯透析与常规透析2种方法治疗前后患者血红蛋白(hemoglobin,Hb)、血浆白蛋白(albumin,MB)、钠(Na+)、C反应蛋白(C-reactive protein,CRP)、心脏射血分数(ejection fraction,EF)值及心功能(NYHA分级)等相关参数的变化并进行比较。结果:联合治疗前后患者血Hb、MB、CRP、EF值差异均具有统计学意义(P<0.05),而血Na+差异无统计学意义(P>0.05)。结论:该联合血液净化方法可纠正体内的炎症状态、改善营养状态、减轻贫血、增加心肌灌注,从而改善心脏功能。 Objective: To investigate the effect of combined technique of dialysis on senium suffering from uremia and heart failure.Methods: From January of 2004 to January of 2009 in blood purification center of our hospital,old patients(n=38) with uremia and heart failure were observed by own control.Patients were treated with a combined technique of dialysis which united with hemodiafiltration(HDF),hemoperfusion(HP) and sequential dialysis for half a year,and the levels of hemoglobin(Hb),albumin(MB),natrium of blood(Na+),C-reactive protein(CRP),ejection fraction(EF) and heart function(NYHA) were tested and compared after 6-month therapy.Results: The levels of Hb,MB,CRP and EF were all improved after therapy(P0.05),and the level of Na+ wasn’t(P0.05).Conclusion: The combined technique of dialysis could adjust the state of inflammation,improve the condition of nutrition,lessen anemia,add heart perfusion,and promote cardiac function.
