非小细胞肺癌MSCT征象与CD44v6表达间相关性研究

摘要 目的:探讨非小细胞肺癌(non-small cell lung cancer,NSCLC)的MSCT征象与CD44v6表达的相关性。方法:对40例经手术病理证实的NSCLC患者术前进行MSCT扫描,并用免疫组织化学法对肿瘤标本进行染色,检测CD44v6在肺癌组织、癌旁组织中的表达。结果:①CD44v6在肿块最大径>3cm的肺癌组织中的表达与≤3cm肺癌组织中的表达无明显差异;MSCT上肿块轮廓出现分叶征、边缘出现短毛刺或棘状突起者,CD44v6阳性表达率明显增高;②CD44v6在淋巴结转移组肺癌中的表达高于无淋巴结转移组;③CD44v6在胸膜受累组肺癌中的表达高于胸膜未受累组;④CD44v6在肺癌组织中的表达显著高于癌旁组织(P<0.05)。结论:NSCLC的MSCT征象与CD44v6表达具有重要关系;CD44v6的异常高表达可能在NSCLC的浸润和转移机制中起重要作用。 Objective:To explore correlation of MSCT appearance and CD44v6 expression in non-small cell lung cancer(NSCLC).Methods:40 cases of NSCLC confirmed by operation and pathology were examined with MSCT before operation.The paraffin embedded tumor samples were assessed immunohistochemically to detect CD44v6 expression in lung cancer tissue and paracancer.Results:① There was no significant difference between tumors of diameter over 3 cm and those of diameter 3 cm or less than 3 cm in CD44v6 expression;The positive rate of CD44v6 expression became obviously high in tumors with deep lobulation,spiculate protuberance and /or shaggy margin signs on MSCT;② CD44v6 expression in tumors with lymph node metastases was significantly higher than that of tumors with no lymph node metastases;③ CD44v6 expression in tumors,which were localized near the pleura with extrapleural fat plane obliterated,was higher than that of tumors without extrapleural fat plane obliterated;④ CD44v6 expression in tumors was significantly higher than that of paratumors(P〈0.05).Conclusion:The high expression of CD44v6 in NSCLC may have a correlation with MSCT appearance.The high expression of CD44v6 may play an important role in NSCLC invasion and metastasis mechanism.
