老年男性性激素水平与骨密度及骨代谢指标的关系 被引量:11

Correlation between serum sex hormone levels and bone minaral density and parameters of bone metabolism in aged men

摘要 目的:探讨老年男性性激素水平与骨密度、骨代谢指标的关系,为老年男性骨质疏松症的防治提供借鉴。方法:收集老年男性105例,年龄为52岁~93岁,分别检测雌二醇(E2)、睾酮(T)、血钙(Ca)、磷(P)、总Ⅰ型前胶原N端肽(PINP)、尿钙、尿钙/肌酐比值,同时测量其左股骨上端的股骨颈、股骨大转子、华氏三角、股骨干及第1至第4腰椎(L1~L4)的骨密度。结果:血清E2水平与尿钙/肌酐比值呈负相关(r=-0.277,P<0.01),与股骨大转子骨密度值呈正相关(r=0.210,P<0.05)。血清T水平与血P呈负相关(r=-0.232,P<0.01),与骨密度无关。血清E2、T水平与其它骨代谢指标均无相关性(P均>0.05)。结论:血清E2和T水平与老年男性的骨代谢存在密切关系,激素替代治疗在老年男性骨质疏松症患者的治疗中具有一定的作用。 Objective:To investigate the correlation between serum sex hormone levels and bone minaral density and parameters of bone metabolism in aged men.Methods:Serum estradiol(E2),testosterone(T),Calcium,Phosphorus,procollagen type I N-terminal propeptide(PINP),Calcium urine Ca/Cr of urine and the BMD(bone mineral density) in orthotope lumbar vertebra,femur neck,greater trochanter,Wards triangle region and shaft of femur were measured in 105 aged 52~93 years patients.Results:E2 levels were negatively correlated with Ca/Cr of urine(r=-0.277,P〈0.01),and were positively correlated with the BMD of greater trochanter(r=0.210,P〈0.05).Serum T levels were negatively correlated with Phosphorus in aged men(r=-0.232,P〈0.01),and there was no correlation between T and the BMD.There was no correlation between E2,T and other parameters of bone metabolism(P〉0.05).Conclusion:Serum E2,T are positively correlated with parameters of bone metabolism in aged men,they can be used as one of the parameters for osteoporosis in aged men.And it is necessary that in therapy of osteoporosis in aged men with the way of hormone replacement therapy.
