骨科无菌手术切口感染的病原菌及耐药率分析 被引量:35

Distribution and Drug Resistance of Bacteria Isolated from Postoperative Infections in Orthopedic Patients

摘要 目的探讨骨科无菌手术切口感染的病原菌种类及其对常用抗菌药物耐药谱等,为临床和医院感染管理提供第一手资料。方法对124例骨科住院患者手术后切口感染的病原菌分布及耐药性进行分析。结果培养出病原菌89株,其中革兰阴性杆菌62株,占69.66%,革兰阳性球菌26株,占29.21%;革兰阴性杆菌以大肠埃希菌(42.70%)、铜绿假单胞菌(12.36%)、肺炎克雷伯菌(8.99%)为主,革兰阳性球菌以金黄色葡萄球菌(19.10%)、表皮葡萄球菌(6.74%)为主,大肠埃希菌以亚胺培南最敏感(100.00%),金黄色葡萄球菌以万古霉素最敏感(100.00%)。结论骨科无菌手术切口术后感染问题值得高度重视,应定期、及时的按照细菌培养和药物敏感性检测结果慎重选用抗菌药物。 OBJECTIVE To study the distribution of pathogens and the extent of drug resistance after orthopedic aseptic operation,and offer the first-hand information to the clinical preventive and therapeutic countermeasures.METHODS Totally 124 inpatients were investigated retrospectively.RESULTS A total of 89 pathogen strains were cultured from the secretion of orthopedic aseptic operation.Among the isolated bacteria,69.66%(62/89)of them were Gram-negative bacilli including Escherichia coli(42.70%),Pseudomonas aeruginosa(12.36%) and Klebsiella pneumoniae(8.99%);and 29.21% were Gram-positive cocci concerning with Staphylococcus aureus(19.10%) and S.epidermidis(6.74%).Among them,the most sensitive antibiotic for E.coli was imipenem(100.00%) and the most sensitive antibiotic for S.aureus was vancomycin(100.00%).CONCLUSIONS Orthopedic aseptic operation should be attached importance to.Clinical doctors should use antibiotics carefully according to the results of bacterial culture and drug-sensitive examination in order to prevent multiple drug-resistant bacteria.
