肾积水患儿病肾肾组织胰岛素样生长因子、内皮素、转化生长因子及单核细胞趋化蛋白表达变化的意义 被引量:4

摘要 目的检测肾积水患儿病肾肾组织胰岛素样生长因子(IGF-1)、内皮素(ET-1)、转化生长因子(TGF-β1)及单核细胞趋化蛋白(MCP-1)的表达变化,探讨其与病肾损害程度的关系。方法收集41例肾积水患儿病肾肾组织标本,并以病肾肾组织结构改变的病理分级(按Elder等标准分为Ⅰ~Ⅴ级)为标准进行分组。8例正常肾组织标本来自肾母细胞瘤周围正常肾组织。免疫组织化学法检测IGF-1、ET-1、TGF-β1及MCP-1在正常肾脏和病肾肾组织内的表达水平,反转录-PCR(RT-PCR)检测IGF-1、ET-1、TGF-β1及MCP-1的基因水平在正常肾脏和病肾肾组织内的表达变化,并与病理分级作相关分析。结果1.IGF-1在正常肾组织呈强阳性表达,在病肾的表达随病理分级的增加逐渐降低;ET-1、TGF-β1及MCP-1在正常肾组织呈弱阳性表达,在病肾的表达随病理分级的增加而升高。2.RT-PCR显示病肾IGF-1明显降低,为0.51±0.03,正常肾为0.97±0.05,差异有统计学意义(P<0.01);病肾ET-1、TGF-β1、MCP-1表达均明显升高,分别为0.41±0.03、0.49±0.05、0.71±0.06,正常肾相应值为0.05±0.02、0.03±0.01、0.09±0.03,差异均有统计学意义(Pa<0.05)。3.病肾IGF-1表达与病理分级呈显著负相关(r=-0.879P<0.01);ET-1、TGF-β1、MCP-1表达与病理分级呈弱正相关,但无统计学意义(P>0.05)。结论小儿肾积水病肾肾组织中IGF-1表达变化可作为临床评定肾损害程度的理想指标之一。 Objective To evaluate the damage degree of hydronephrotic kidneys(HnKs) by detecting insulin-like growth factor(IGF-1),endothelin(ET-1),transforming growth factor(TGF-β1) and monocyte chemoattractant protein(MCP-1) expression in children with hydronephrosis.Methods The HnKs specimens of 41 cases were collected and divided into 5 groups according to pathologic grades(Ⅰ-Ⅴgrades).Eight renal samples from the normal part of kidney of Wilm's tumor were selected as control group.Immunohistochemistry was applied to determine the expressions of IGF-1,ET-1,TGF-β1 and MCP-1.The mRNA levels of IGF-1,ET-1,TGF-β1 and MCP-1 were determined by semi-quantitative reverse transcription-polymerase chain reaction(RT-PCR).The correlations of pathologic grades with mRNA levels of IGF-1,ET-1,TGF-β1 and MCP-1 were analyzed by Spearman's test.Results 1.The IGF-1 expression of normal kidney was strongly positive and that of HnKs was gradually decreased with increasing of pathologic grades.Whereas the expressions of ET-1,TGF-β1 and MCP-1 of the normal kidney were weakly positive and that of HnKs was gradually rised with increasing of pathologic grades.2.In healthy and HnKs,the relevant parameters by RT-PCR determination were as follows:IGF-1 was(0.97±0.05)vs(0.51±0.03)(P〈0.01);ET-1 was(0.05±0.02)vs(0.41±0.03),TGF-β1 was(0.03±0.01)vs(0.49±0.05)and MCP-1 was(0.09±0.03) vs(0.71±0.06),respectively(Pa〈0.05).3.There was closely negative correlation between IGF-1 level and pathologic grades(r=-0.879 P〈0.01);whereas there was positive correlation between the levels of ET-1,TGF-β1,MCP-1 and pathologic grades but with no significance(P〉0.05).Conclusion The IGF-1 expression may serve as an ideal marker to evaluate damage level of HnKs in children with hydronephrosis.
