摘要
目的了解长沙市医保退休人员代谢综合征患病情况,为该人群此类疾病的防治提供依据。方法收集2008年6-12月在湖南省军区机关医院体检的长沙市医保退休人员资料共4142例,按中华医学会糖尿病分会代谢综合征(Metabolic syndrome,MS)诊断标准进行诊断,分析计算MS的患病情况,各单项代谢异常检出率等。结果受检者中753例符合MS诊断标准,患病率为18.18%;男性患病率高于女性(χ2=27.20,P<0.01),分别为22.35%和15.84%。各年龄组患病率以60～69岁组为最高(26.95%);其余各组为50～59岁组10.09%,70～79岁组22.54%,80岁以上组10.76%。各单项代谢异常的检出率从高到低依次是血压增高(39.38%)、血脂异常(34.96%)、超重和/或肥胖(34.87%)和糖调节受损(14.82%)。代谢异常的组合以"高血压+血脂异常+超重和/或肥胖"检出率最高(7.22%),构成比为39.71%。结论长沙市医保退休人员MS患病水平处于全国中等水平,60～69岁是其高发年龄段,本病已经成为长沙市中老年人群的常见病和多发病,应及早进行健康教育并采取有效措施,以降低心血管事件的发生。
Objective To know the prevalence of metabolic syndrome （MS） among the retired with medical insurance in Changsha,and to provide the evidence for prevention and treatment of the disease in these people.Methods We collected 4,142 cases by interviewer-administered questionnaire,biophysical assessment,and biochemical examination.All of them were the retired with medical insurance and took physical examination in Hospital of Hunan Military Area Command during June to December of 2008.According to the working definition and diagnosis standard on MS initiated by the Diabetes Chapter of the Chinese Medical Association,the cases were diagnosed by MS.Age and gender specific prevalence rates and the general prevalence rate for MS patients were analyzed,as well as prevalence rates of each single metabolic disorder test.Results Among the 4,142 examinees,753 met the diagnosis standards for MS,and the general incidence of MS was 18.18%.The MS prevalence of the males （22.35%） was higher than that of the females （15.84%）,（χ2=27.20,P0.01）.The prevalence of MS was the highest （26.95%） in the age group of 60～69 years,followed by 10.09% in the age group of 50～59 years,22.54% the age group of 70～79 years,and 10.76% in the age group of over 80 years.According to the positive rates,the single metabolic disorder tests in ascending order were hypertension （39.38%）,dyslipidemia （34.96%）,obesity and/or overweight （34.87%）,and impaired glucose regulation （14.82%）.The MS patients with ＂hypertension ＋obesity and/or overweight） ＋ dyslipidemia＂ had the highest prevalence rate（7.22%）and accounted for 39.71%.Conclusions The MS prevalence rate of the retired with medical insurance in Changsha is at a moderate level in China.MS has become a common and frequently-occurring disease among middle-aged and old population,especially in 60～69 years old people.The health education concerned to MS and primary and secondary prevention should be carried out as early as possible in order to reduce the prevalence of cardiovascular events.
