摘要
目的探讨肘关节内外侧联合入路双钢板内固定术治疗C型肱骨髁间骨折的临床疗效。方法采用肘关节内外侧联合入路切开复位双钢板内固定术治疗C型肱骨髁间骨折28例,骨折按AO/ASIF分型:C1型11例,C2型9例,C3型8例。结果28例患者均获得随访,平均随访6～37(18.9±3.3)个月,骨折全部愈合,平均愈合时间3～6(4.2±1.1)个月。采用改良Casse-baum评分系统评定肘关节疗效,优18例,良6例,可3例,差1例,优良率85.7%。结论肘关节内外侧联合入路双钢板内固定术治疗C型肱骨髁间骨折,避免了关节周围组织的过多损伤,保全了伸肘装置且固定牢靠。此外,术后可早期行动,能锻炼,最大程度地恢复肘关节功能,疗效满意。
Objective To investigate double plates internal fixation with elbow joint interior and exterior lateral combination approach for the treatment of C type of intertrochanteric fracture of humerus. Methods 28 cases of C type of intertrochanteric fracture of humerus, including 11 cases C1-type, 9 cases C2-type, and 8 cases C3-type, according to AO/ ASIF fracture classification,were treated with double plates internal fixation with elbow joint interior and exterior lateral combination approach. Results All 28 patients were followed up for an average of 6 - 37 （ 18.9 ± 3.3） months, fractures were all healed, with an average healing time of 3 - 6 （4.2 ± 1.1 ） months. According to Cassebaum Scoring system for elbow joint function, 18 cases were excellent,6 cases good, and 3 cases median, and 1 case poor, with an excellence rate of 85.7%. Conclusion By treating C type of intertrochanteric fracture of humerus with double plates internal fixation in an approach of elbow joint interior and exterior lateral combination, excessive joint damage surrounding tissue can be avoided, elbow stretching device be protected, and fixation assured. In addition, early functional exercise can recuperate elbow joint function and assure satisfied therapeutic effects.
