摘要
目的探讨上消化道重复癌的外科治疗方法及疗效。方法1999年8月至2009年8月收治食管胃重复癌22例,全组均行手术治疗,一期成功完成根治性全胃切除,食管次全切除结肠代食管、空肠代胃重建消化道17例。近段胃大部切除,食管次全切除,结肠代食管3例,2例手术探查。结果全组无围手术期死亡,术后并发颈部吻合口瘘2例,严重心律失常1例,呼吸衰竭1例,肺不张1例,均行保守治疗后痊愈,20例获得随访,1、3、5、10年生存率分别为95%,55%,30%.5%(19/20,11/20,6/20,1/20)。结论食管次全切除,全胃切除或近端胃大部切除、结肠代食管、空场代胃重建消化道是治疗上消化道重复癌安全有效的外科治疗方法。
Objective To investigate the operative technique and operative effects of esophageal cancer associated with stomach gastric cancer. Methods Twenty-two cases suffered from esophageal cancer associated with stomach tumor had undergone surgical treatment from July 1999 to July 2009. The diagnosis, radical operative methods, and the results were studied. Results Seventeen patients underwent a partial esophageetomy reconstructed with colon and a total gastrectomy reconstructed with p or Braun shaped jejunum. Three patients underwent a partial esophagectomy with of the colon and a total gastrectomy reconstruction with gastrectomy jejunum and only two patients the reconstructed colon failed because of dysplasia of ＂arc of Riolan.＂ Result： There was no preoperative mortality, six patients had post-operative anastomodc leakage, one patient had cardiac arrhythmia, and one patient had a non ventilating lungo All the post-operative complications were cured. Twenty patient had a followed up at 1, 3, 5 and 10 year intervals with a survival rate of 95%, 55%, 30% and 5% respectively. Conclusion Synchronous a partial esophagectomy reconstruction with colon reconstruction and total gastrcctomy or complete gastrectomy with reconstruction with p-shaped or Braun shaped jejunum was safe and effective radical operation for synchronous esophageal cancer associated with gastric tumot.
