摘要
目的探讨磁共振胰胆管造影(MRCP)联合动态增强扫描(LAVA)对癌性梗阻性黄疸鉴别诊断的价值。方法106例癌性梗阻性黄疸患者行MRI、MRCP和LAVA检查,进行诊断及鉴别诊断,并与病理结果进行对比分析。结果106例癌性梗阻性黄疸中,胆管癌45例,胰头癌22例,胆囊癌18例,壶腹癌12例,十二指肠癌9例。不同来源的肿瘤引起胆管系统的改变及强化方式有一定的差异。本组癌性梗阻性黄疸MRI诊断与手术病理结果符合率约为92.5%。结论MRCP和LAVA的联合应用对癌性梗阻性黄疸的鉴别诊断具有重要价值,其影像学结果对临床治疗决策有指导意义。
Objective To explore the value of combination of magnetic resonance eholangiopancreatography （MRCP） and liver acquisition with volume acceleration （ LAVA ） in the differentia/ diagnosis of cancerous obstructive jaundice. Methods MRI, MRCP and LAVA were used to identify the location and size as well as infiltrating circumscription of carcinoma in 106 patients. The information for diagnosis and differential diagnosis of cancerous obstructive jaundice was obtained by analyzing these images. The results were compared with pathological findings. Results Among 106 cases, there were 45 cases of cholangiocarcinoma ,22 cases of carcinomas of head of pancreas, 18 cases of carcinomas of gallbladder, 12 cases of ampullary carcinoma,9 cases of duodenal carcinoma. The changes of biliary tract and the styles of contrast caused by different cellular source carcinoma were differential. MRI, MRCP and LAVA had coincidence of 91.4% with pathological findings in diagnosis of cancerous obstructive jaundice. Conclusion MRCP combined with LAVA could definitely demonstrate the preoperative organ metastasis and infiltrating circumscription of carcinoma, especially blood vessel invasion, it can play an important role in differential diagnosis of cancerous obstructive jaundice, and their images are helpful to selection of the therapeutic regimen.
