摘要
目的:探讨外侧裂小骨窗入路显微手术治疗超早期高血压性基底节区出血的疗效,提高高血压脑出血的显微外科治疗水平。方法:对67例超早期高血压性基底节区出血采用小骨窗开颅,经外侧裂入路,显微镜下清除血肿。结果:术后24h内复查CT了解血肿清除情况,58例血肿清除90%以上,9例血肿清除80%以上。随访6个月,按GOS恢复标准:Ⅴ+Ⅳ级68%,Ⅲ级19%,Ⅱ级3%,Ⅰ级(死亡)10%。结论:外侧裂小骨窗入路显微手术创伤小,手术显露满意,止血可靠,能有效降低颅内压,是高血压性基底节区出血的有效手术方式之一。
Objective: To explore the effectiveness of microsurgical treatment via lateral fissure approach for hypertensive basal ganglia hematoma.Methods: A total of 67 patients with hypertensive basal ganglia hemorrhage were performed under small bone flap eraniotomy through lateral fissure.The hematoma was cleared under microscope.Results: The CT reexamination within 24 hours postoperation showed that hematoma was removed over 90% in 58 cases and over 80% in 9 cases.Follow-up assessment according to the Glasgow Outcome Scale(GOS)for 6 months revealed that the Ⅴ and Ⅳdegree was 68%, Ⅲdegree was 19%, Ⅱdegree was 3% and Ⅰ degree was 10%. Conelusion:Mierosurgical treatment via lateral fissure approach has the advan rage of minimal invasion,good operational field exposure, reliable hemostasis, decreasing intracranial pressure effectively, and it is one of the effective methods for hypertensive basal ganglia hematoma.
