关节镜下联合持续冲洗对骨性关节炎的康复影响 被引量:5

NO Level in Synovial Fluid of Different Treatment in Patients with OA after Knee Arthroscopy Debridement

摘要 目的研究关节液中NO水平的变化,探讨关节镜下联合持续冲洗对膝关节骨性关节炎的康复影响。方法将确诊为膝关节骨性关节炎的32例随机分成两组,每组16例。一组在膝关节镜手术后用3000ml的生理盐水持续6h冲洗关节腔;一组不行关节腔冲洗;于术前﹑术后24h,1、2、4周分别抽取2ml的膝关节液,检测NO相应浓度。于不同时段对其疗效进行评分。结果持续冲洗组与非持续冲洗组均能使关节液中的NO浓度水平下降。术后2周内,持续冲洗组和非持续冲洗组的NO检测结果有显著不同,前者优于后者。术后4周,持续冲洗组和非持续冲洗组的NO检测结果不再有显著差异。疗效评分:术后2周持续冲洗组优于非持续冲洗组;术后4周评分比较两组无明显差异。但两组患者术前和术后4周比较有明显差异。结论关节镜下联合持续冲洗可在较短时间内使关节液中NO的水平下降,迅速有效地改善症状,对膝关节骨性关节炎的康复有积极影响。 Objective To compare the NO level in synovial fluid of different treatment in patients with OA after knee arthroscopy debridement. Methods Thirty-two OA patients were divided into two groups, 16 OA patients in eaeh group. In one group patients the joint was irrigated for 6 hours with 3000 ml NS after knee arthroscopy debridement while in the other group the joint was not irrigated after knee arthroseopy debridement. Two ml synovial fluid were obtained in all patients before operation, and 24 hours, 1 week, 2 weeks,4 weeks after operation and the NO level was detected. The therapeutic effeet was scored at different period of the time. Results The irrigation group and the no-irrigation group both decreased the NO level. Two weeks after operation, the irrigation group and the no-irrigation group had significant difference in the NO level. The former was better than the latter. Four weeks after operation, the result had no significant difference between the irrigation group and the no-irrigation group. Scores of the therapeutic effeet: the irrigation group was better than the no- irrigation group at the second week after operation; the score at the fourth week after operation had no significant difference. But 2 groups both had significant difference before and after operation. Conclusion The continuous irrigation after knee arthroscopy debridement can decrease the NO level in synovial fluid in short time, improve the symptoms rapidly, whieh faeilitates the recovery of the knee OA.
