摘要
目的总结血管内治疗椎基底动脉冗扩(VBD)致三叉神经痛的经验。方法对1例VBD致三叉神经痛病例采用多支架植入治疗,并对比分析手术前后MRI表现,探讨其可能的治疗机制。结果术后病人三叉神经痛明显改善,冗扩血管段支架内血流增快,支架外血流淤滞,血栓形成。结论支架植入后的局部血液动力学改变可能是三叉神经痛缓解的基础,支架成形术治疗VBD致三叉神经痛值得尝试。
Objective To analyze the clinical experiences in endovascular treatment for trigeminal neuralgia caused by vertebrobasilar dolichoectasia （VBD）. Methods A patient with trigeminal neuralgia caused by VBD was treated by multiple-stent placement. Results of MRI were compared between pre-operation and post-operation, and the possible mechanisms of the treatment were analyzed. Results The postoperative symptom of neuralgia was remarkably relieved. The blood flow within the stent was speeded up, while blood stasis and thrombosis outside the stents was noticed post-operation. Conclusion The amelioration of neuralgia may due to focal hemodynamic change after stent implantation. Stent angioplasty is worthy to be considered in the treatment of trigeminal neuralgia caused by VBD.
