摘要
探讨尿微量白蛋白(MA)、尿转铁蛋白(TRU)、尿IgG蛋白(IgG)、尿α1微球蛋白(α1MG)在糖尿病肾病早期诊断中的意义。利用速率散射免疫比浊法测定了61例2型糖尿病患者和66名健康对照者4种尿特定蛋白的含量。对66名对照组4种尿特定蛋白进行联合检测,结果无1例阳性;糖尿病组4种尿特定蛋白MA、TRU、IgG、α1MG所测到的阳性率分别为60.66%、50.28%、44.26%、47.54%,并且糖尿病组4种尿特定蛋白的测定值显著高于对照组的测定值(P均〈0.05);研究表明糖尿病组尿特定蛋白与年龄有相关性,年龄越大阳性率越高(P〈0.01);但是不同年龄段对这些特定蛋白的易感性有所不同;病程早期,即在最初的发病的5年内,尿特定蛋白检测到的结果高于发病6~10年的患者,10年以后逐渐增高;尿特定蛋白与糖尿病病程关系密切,病程越长尿特定蛋白阳性率也随着越高(P〈0.05)。结论:测定尿MA、TRU、IgG、α1MG有助于糖尿病肾病的早期预防和早期诊断。
To investigate the early diagnosis value of urinary MA,TRU,IgG,and α1MG in diabetic nephropathy.Morning middle period urine samples were collected from 61 diabetics patients of type II and 66 healthy people which ages and gender matched with patients group,the levels of four urinary MA,TRU,IgG,and α1MG were detected by immunoturbidimetry of rate scatter.The results showed that the positive rates of four urinary proteins in the patients were 60.66%,50.28%,44.26% and 47.54% respectively.The levels of urinary MA,TRU,IgG,and α1MG in patients are significant higher than that of control group(P〈0.05).The positive rate of four urinary proteins in more than 40 years old patients was significant higher than that of young patients(P〈0.01).The levels of four urinary proteins in patients in the course of first 5 years were higher than that of patients in the course of 6 to 10 years(P〈0.05).The positive rate of four urinary proteins was increased with the course of diseases.The results indicate that the detection of urinary MA,TRU,IgG and α1MG are helpful in the early diagnosis and prevention of diabetic nephropathy.
