2型糖尿病骨质疏松的发病率及其相关因素分析 被引量:23

摘要 目的分析2型糖尿病对骨质疏松发病率的影响及骨质疏松与年龄、体重指数、糖尿病的相关性。方法①选择102例2型糖尿病患者及146例对照组病例,测量其骨密度(BMD)值,采用病例对照法分析2型糖尿病对骨质疏松发病率的影响;②运用Logistic多元回归的方法分析糖尿病、年龄、性别、体重指数对骨质疏松的影响;③对糖尿病、年龄、体重指数与骨质疏松的相关性进行分析。结果①2型糖尿病组的骨质疏松发病率为55.9%,对照组为43.2%,糖尿病组高于对照组;②骨质疏松与年龄及糖尿病正相关,与体重指数负相关;糖尿病、性别、年龄及体重指数对骨质疏松的影响均有统计学意义;③糖尿病组的年龄、体重指数与骨质疏松的相关性均有统计学意义;④对照组的年龄对骨质疏松的相关性影响有统计学意义;⑤ward’s三角区为两组病例骨质疏松共同的好发部位。结论①2型糖尿病是骨质疏松的重要影响因素,与骨质疏松正相关;②2型糖尿病患者应保持适当的体重指数,以延缓骨质疏松的发生、减轻骨质疏松的程度;③无论有无糖尿病,中老年人均应定期检查BMD,以早期发现、早期治疗骨质疏松,降低由骨质疏松导致的腰椎、股骨骨折发生率,提高老年人生活质量。④ward’s三角区对骨质疏松最敏感,可作为首选检查部位。 Objective To investigate the effect of type 2 diabetes mellitus on incidence of osteoporosis and the correlation of osteoporosis with age, body mass index (BMI) and diabetes. Methods (1)The patients with type 2 diabetes mel]itus of 102 cases and control patients of 148 cases were selected to measure the Bone Mineral Density (BMD), and the case-control study was used to analyze the effect of type 2 diabetes mellitus on incidence of osteoporosis ; (2)The logistic muhiple regression was used to analyze the influence of diabetes, age, sex and BMI on osteoporosis;(3)The correlation analysis of diabetes, age, BMI and osteoporosis was performed. Results (1)The group of type 2 diabetes mellitus had a higher osteoporosis incidence than the control group, ie. 55.9% and 43.2%, respectively; (2)The positive correlation of osteoporosis with age and diabetes was found out, with its negative correlation with BMI; the effect of diabetes, age, sex and BMI on osteoporosis had a statistical significance; (3)In the group of diabetes, the correlation of age and BMI with osteoporosis had a statistical significance; (4) In the control group, only age had a statistieal osteoporosis relevance; (5)The Ward's Triangle Area was the predilection site of osteoporosis. Conclusion (1)The type 2 diabetes mellitus is an important risk factor; (2)The patients with type 2 diabetes mellitus should maintain a proper BMI; (3) The regular examination of BMD should be performed for the middle-aged and elderly people in order to treat osteoporosis and prevent its complications; (4)The ward's triangle area was regarded as the first choice of examination for early diagnosis of osteoporosis.
