甲泼尼龙治疗重症急性胰腺炎疗效分析 被引量:4

The efficacy analysis of methylprednisolone treatment of severe acute pancreatitis

摘要 目的探讨甲泼尼龙对重症急性胰腺炎的治疗作用。方法回顾分析辽宁医学院附属第三医院2004年1月至2007年12月收治的重症急性胰腺炎患者62例,其中常规治疗组32例(对照组),甲泼尼龙治疗组30例(治疗组)。对比两组患者腹痛、腹胀缓解时间,血、尿淀粉酶恢复正常时间,腹水消退时间,病死率,并发症发生率,住院时间等,以评价甲泼尼龙治疗重症急性胰腺炎的临床疗效。结果治疗组腹痛、腹胀缓解时间,血、尿淀粉酶恢复正常时间,腹水消退时间,住院时间较对照组明显缩短(P<0.05);治疗组的主要并发症如ARDS,MODS,胰腺假性囊肿和病死率明显低于对照组(P<0.05)。但是,两组患者之间合并胰周脓肿及肝功能不全、肾功能不全、心功能不全恢复正常时间的比较,差异无统计学意义(P>0.05)。结论甲泼尼龙对重症急性胰腺炎有明显临床疗效,能够降低病死率,减少并发症,缩短住院时间。 Objective To investigate the therapeutic effects of methylprednisolone(MP)in the treatment of the patients with severe acute pancreatitis(SAP).Methods 62 paitents with severe acute pancreatitis in our department from january 2004 to december 2007 including two groups:convention group(control group,n=32)and methylprednisolone group(treatment group,n=30).To reviewed analyzed and compared with the relieve time of abdominal pain abdominal distention,the regression time of abdominal dropsy,the hospital stay(day),the morbidity of complications,the case fatality,the restore time of hemodiastase,urinary amylase.Results The relieve time of abdominal pain abdominal distention,the regression time of abdominal dropsy,the hospital stay,the restore time of hemodiastase,urinary amylase in treatment group were markedly shorter than those in control group(P〈0.05),the case fatality,the morbidity of such complications as ARDS,MODS,pancreatic pesudocyst,in treatment group was markedly lower than that in the control group(P〈0.05).But the restore time of hepatic inadequacy,the renal inadequacy,the cardiac insufficiency and the parapancreatic abscess between two groups had no differences(P〉0.05).Conclusions The medicine methylprednisolone has good therapeutic effects in the treatment of the patients with SAP.It can decrease the case fatality,reduce the hospital stays and complications.
