破碎型腰椎间盘疝出和退变型腰椎间盘突出的X线及MRI表现 被引量:2

X-ray and MRI Findings or Reptured Lumbar Disc Herniation and Degenerative Lumbar Disc Protrusion

摘要 目的:探讨破碎型腰椎间盘疝出和退变型腰椎间盘突出的X线及MRI表现。方法:回顾性分析110例腰椎间盘突出症,依术中所见分为两组:破碎型腰椎间盘疝出组和退变型腰椎间盘突出组,比较两者的X线及MRI表现。结果:72例破碎型椎间盘疝出组有48例椎体骨质增生,21例病变节段椎间盘内低信号,5例出现椎间盘上下终板信号异常,35例病变椎间盘上下椎体信号异常。38例退变型腰椎间盘突出组有34例椎体骨质增生,32例病变节段椎间盘内出现低信号,12例病变椎间盘上下终板信号异常,12例病变椎间盘上下椎体信号出现异常。采用两组χ2检验,两组比较有统计学意义(P<0.05)。结论:X线及MRI中T2加权像对区分破碎型腰椎间盘疝出和退变型腰椎间盘突出有一定的价值,从而更好指导腰椎间盘突出症的治疗。 Objective To investigate the X - ray and MRI findings of raptured lumbar disc herniation and degenera- tive lumbar disc protrusion. Methods One hundred and ten cases of lumbar disc herniation were divided into 2 groups by operative findings: ruptured lumber disc herniation and degenerative lumbar disc protrusion. The X - ray and MRI findings of the two groups were compared. Results In the 72 cases of the ruptured lumbar disc herniation group, 48 cases were found to have a different bone hyperplasia, 21 cases had a low signal performance in the pathological disc, 5 cases had lamina tenninalis signal abnormalities at the top and bottom laminae teruainalis of the pathologic disc, 35 cases had a vertebral signal anomaly at the top and bottom vertebrae of the pathological disc. In the 38 cases of the degenerative lumbar disc protrusion gToup, 34 cases were found to have a different bone hyperplasia, 32 cases had a low signal, per- formance in the pathological disc, 12 cases had lamina terminalis signal abnormalities at the top and bottom laminae ter- minalis of the pathologic disc, 12 cases had a vertebral signal anomaly at the top and bottom vertebrae of the pathologic disc; there were statistical differences( P 〈 0.05). Conclusion X - ray and MRIT2WI has a good diagnostic value in differentiation of ruptured lumbar disc herniation and degenerative lumbar disc protrusion, providing a good guide in the treatment of lumbar disc herniation.
