摘要
目的:探讨首发精神分裂症患者药物治疗前后血清皮质醇和脑源性神经营养因子(BDNF)水平的变化。方法:30例首发精神分裂症患者(研究组)给予利醅酮治疗8周,于治疗前后检测血清皮质醇、BDNF水平;对30名正常人采用相同方法进行检测,作为对照。采用阳性和阴性症状量表(PANSS)评定治疗前后的精神症状。结果:治疗8周,研究组皮质醇、BDNF水平较治疗前明显下降(t=2.78、2.85,P均<0.01),BDNF水平与皮质醇水平呈正相关(r=0.415,P<0.05)。治疗前研究组皮质醇及BDNF水平与PANSS总分、阳性因子分、一般精神病理分呈显著正相关(r=0.371、0.425、0.406,P均<0.05;r=0.389、0.432、0.411,P均<0.05);但与阴性因子分无显著相关(r=0.337、0.342,P>0.05);治疗后研究组皮质醇及BDNF水平分别与PANSS总分及各因子分无显著相关(r=0.341、0.335、0.338、0.336,P均>0.05;r=0.349、0.332、0.350、0.353,P均>0.05)。结论:首发精神分裂症患者的精神症状发作时,存在生物学应激,表现为皮质醇水平升高,机体处于代偿阶段,BDNF表达上调,治疗后应激水平降低,表现为皮质醇水平下降,BDNF也相应降低。
Objective: To explore the change of levels of serum cortisol and brain-derived neurotrophic factor(BDNF) before and after medicine treatment in first-episode inpatients with schizophrenia. Method: The levels of serum cortisol and BDNF were measured before and after 8 weeks medicine treatment in 30 first- episode inpatients (study group),30 normal controls were measured by the same way. The symptoms were in- vestigated with the positive and negative syndrome scale (PANSS) before and after treatment . Results:The levels of serum cortisol and BDNF in first-episode inpatients significantly decreased after 8 weeks medicine treatment (t = 2.78,2.85, all P 〈 0.01 ). The levels of serum cortisol and BDNF were significantly positively correlated in first-episode inpatients ( r = 0. 415 ,P 〈 0.05 ). The levels of serum cortisol and BDNF of the first- episode inpatients before treatment were significantly related to the total PANSS scores, positive symptom scores, general psychopathology symptom scores( r = 0. 371,0.425,0. 406,0. 389,0.432,0.411, all P 〈 O. 05 ), while were not significantly related to the negative symptom scores ( r = O. 337,0.342, all P 〉 0.05 ). After treatment, the levels of serum cortisol and BDNF of the inpatients were not significantly related to the total PANSS scores and other scores ( r = 0. 341,0. 335 ,0. 338,0. 336,0. 349 ,0. 332,0. 350,0. 353 , all P 〉 0.05 ). Conclusion: Schizophrenia patients reveal biological stress reactions at seizure of psychotic symptoms with increased levels of cortisol and levels of BDNF. The biological stress reactions decrease with the level of cortisol and of BDNF decreased.
