原发性肾病综合征患儿血清白细胞介素-13与激素耐药的相关性 被引量:5

Relationship between Interleukin-13 and Steroid-Resistant in Children with Primary Nephrotic Syndrome

摘要 目的探讨原发性肾病综合征(PNS)患儿血清IL-13蛋白水平及外周血单个核细胞(PBMC)IL-13mRNA表达与激素耐药的相关性,以期阐明IL-13在PNS激素耐药中的可能机制。方法分别采用ELISA及RT-PCR法测定40例健康儿童及39例激素敏感型肾病综合征(SSNS)、27例激素耐药型肾病综合征(SRNS)患儿血清IL-13蛋白水平及PBMC中IL-13mRNA表达水平。采用双缩脲法检测上述患儿24h尿蛋白。全自动生化分析仪检测血清总胆固醇(Chol)、三酰甘油(TG)、低密度脂蛋白(LDL)、总蛋白(TP)、清蛋白(Alb)。采用SPSS13.0软件比较各组间差异。结果1.与健康对照组比较,治疗前SSNS、SRNS患儿血清IL-13蛋白水平及PBMC中IL-13mRNA表达均明显增高(Pa<0.05);与SSNS患儿比较,SRNS患儿血清IL-13蛋白水平及PBMC中IL-13mRNA表达均增高(Pa<0.05);SSNS、SRNS患儿血清Alb、TP、Chol、TG水平及24h尿蛋白量比较均无统计学意义(Pa>0.05),SRNS患儿血清中LDL水平较SSNS患儿高(P<0.05)。2.经正规激素治疗8周后,SRNS组血清IL-13蛋白水平及PBMC中IL-13mRNA表达均高于SSNS组及健康对照组(Pa<0.05);SSNS组血清IL-13蛋白水平及PBMC中IL-13mRNA表达与健康对照组比较均无显著性差异(Pa>0.05)。3.治疗后SSNS组IL-13蛋白水平及PBMC中IL-13mRNA表达量较治疗前均明显下降(Pa<0.05)。而治疗前、后SRNS组IL-13蛋白水平及PBMCmRNA表达量均无明显变化(Pa>0.05)。结论PNS激素耐药与IL-13蛋白水平及PBMCmRNA表达明显升高有关,IL-13的过度分泌在SRNS的发生、发展中发挥一定作用。调节血清IL-13蛋白水平及表达可能是治疗SRNS的新途径。 ObjectiveTo explore the relationship between serum concentration of interleukin(IL)-13,expression of peripheral blood monouclear cells(PBMC) IL-13 mRNA and steroid-resistant in children with primary nephrotic syndrome(PNS).To clarify the possible mechanism about IL-13 in PNS.MethodsThirty-nine children with steroid-sensitive nephrotic syndrome(SSNS) and 27 children with steroid-resistant nephrotic syndrome(SRNS) were enrolled in the study(patients group).Forty healthy children were selected as healthy control group after physical examination.The serum concentration of IL-13 was measured by ELISA.The expression of IL-13 mRNA was detected with RT-PCR method.The amount of 24 h urine protein was measured by biuret.The serum concentration of cholesterol(Chol),triglyceride(TG),low density lipoprotein(LDL),total protein(TP),albumin(Alb) were measured by automatic biochemistry analyzer.The differences of different groups were compared with SPSS 13.0 software.Results1.The serum protein concentration of IL-13,expression of PBMC IL-13 in SSNS and SRNS was significantly higher than those of healthy control group(Pa〈0.05).The serum concentration of IL-13,expression of PBMC IL-13 in SRNS was higher than those of SSNS(Pa〈0.05).The comparison of the amount of 24 h urine protein,Chol,TG,TP,Alb between SSNS and SRNS was not statistically significant(P〉0.05).But the serum concentration of LDL in SRNS was higher than that of SSNS(P〈0.05).2.After regular corticosteroid hormone therapy,the serum concentration of IL-13 and expression of PBMC IL-13 mRNA in SRNS were significantly higher than those of SSNS and healthy control group(Pa〈0.05).The comparison between SSNS and healthy control was not statistically significant(P〉0.05).3.The serum concentration of IL-13 and expression of PBMC mRNA in SSNS were significantly lower after therapy than before(P〈0.05).But in SRNS,there were no differences before and after therapy(P〉0.05).ConclusionsSRNS was associated with the elevated serum concentration of IL-13,expression of PBMC IL-13 mRNA.Too more IL-13 serum concentration and expression may play some role in the onset and development of SRNS.Regulation of IL-13 serum level maybe a new therapeutic method for SRNS.
