摘要
[目的]分析研究血清胱抑素C(CystatinC,Cys-C)与肾性贫血时红系参数变化之间的关系,评价Cys-C在肾性贫血诊断中的临床意义。[方法]采用Beckman coulter LH750全自动血液分析仪对60名健康儿童和69名肾性贫血患儿的全血细胞进行分析。按照贫血的严重程度,将试验组患儿分为三组。使用微粒子增强免疫比浊法、酶法和苦味酸法分别检测三组患儿血清中的Cys-C、尿素氮(BUN)和肌酐(Scr)的水平。分析比较对照组和三组患儿的红系参数、Cys-C、BUN和Scr之间的关系。[结果]肾性贫血患儿的Cys-C的水平均显著高于正常儿童(P〈0.05),轻度贫血患儿的BUN和Scr的水平与正常儿童相比,无显著差异。中、重度贫血患儿的BuN和Scr的水平明显高于正常儿童。三组患儿的红系参数、血清Cys-C、BUN和Scr水平之间差异有显著性(P〉〈0.05或P〈0.01);患儿的Cys-C、BUN和Scr与血红蛋白(Hb)之间有着很好的相关性。Cys-C与Scr、BUN在轻度肾性贫血患儿的异常检出率方面差异有显著性(P=0.00,X^2=17.30)。[结论]肾性贫血患儿的Cys-C与贫血程度密切相关,尤其对于提示早期贫血的作用比BUN和Scr更加灵敏。
[Objective] To study the relation between serum cystatin C(Scc) and parameters of red blood cells of renal anemia patients and to evaluate the role of Scc in renal anaemia. [Methods] Hemograms of 69 renal anemia children and 60 healthy children were detected. According to the concentration of hemoglobin, the patients were divided into three groups. Scc, BUN, Scr of renal anemia patients and healthy children were detected by immunoturbidimetry, enzymic method and trinitrophenol method. The relation between serum Scc and parameters of red blood cells was studied. [Results] The level of Scc of renal anemia patients was obviously higher than that of healthy children( P 〈0.05). There was no significant difference in the levels of Scr and BUN between the low-grade anemia patients and healthy children. The levels of Scr and BUN of the mid-grade and grave-grade anemia patients were obviously higher than those of healthy children. There were significant differences in the levels of Scc, Scr and BUN among the three groups( P〈0.05 or P 〈0.01 ). There was high correlation between the concentration of hemoglobin and the levels of Scc, Scr and BUN. There were sig- nificant differences in the detection rate of Scc, Scr and BUN in renal anemia patients. [Conclusion] The level of Scc of renal anemia patients is related closely to the degree of anemia. Especially for the diagnosis of early a- nemia, Scc is more sensitive than BUN and Scr.
