原发性高血压患者颈动脉内膜-中层厚度与肾动脉阻力指数的相关性研究 被引量:7

CORRELATION BETWEEN CAROTID ARTERIAL INTIMA-MEDIA THICKNESS AND RENAL ARTERIAL RESISTANCE INDEX IN ESSENTIAL HYPERTENSIVE PATIENTS

摘要 目的探讨原发性高血压患者颈动脉内膜-中层厚度(intima—media thickness,IMT)与肾动脉阻力指数(resistance index,RI)、搏动指数(pulse index,PI)的相关性,及其对高血压患者的临床意义。方法原发性高血压患者102例,除外严重心、脑、肾等靶器官并发症;22例健康者为对照。研究对象均以彩色多普勒超声诊断仪测量双侧颈总动脉起始部、颈动脉分叉处,颈内、外动脉的IMT及双肾动脉RI、PI,同时检测血清肌酐、尿素等生化指标,比较原发性高血压患者颈动脉IMT均值与肾动脉RI、PI的关系,肾动脉RI、PI与肌酐、尿素的关系,原发性高血压组颈动脉IMT,肾动脉RI、PI与对照组的差异,颈动脉IMT、肾动脉RI、PI与高血压病程的关系。结果原发性高血压组肾动脉RI、P1值明显高于对照组,有显著性差异;原发性高血压患者颈动脉IMT与肾动脉的RI、PI的相关系数r分别为0.49、0.26,P〈0.05;随着病程延长,原发性高血压患者颈动脉IMT、肾动脉RI、PI均有增加趋势,且有显著性差异;原发性高血压患者肾动脉RI与肌酐、尿素的相关系数r分别为0.37、0.20,P值均〈0.05;原发性高血压患者肾动脉PI与肌酐、尿素的相关系数r分别为0.41、0.20,P值均〈0.05。结论①原发性高血压患者颈动脉IMT与肾动脉RI、PI均呈正相关;②随着高血压病程的增加,高血压患者颈动脉IMT、肾动脉RI、PI均呈增高趋势;③原发性高血压患者肾动脉RI、PI与肌酐、尿素均呈正相关;④原发性高血压组颈动脉IMT,肾动脉RI、PI均高于正常对照组。 Objective To observe the correlation among carotid arterial intima-media thickness(IMT) ,renal arterial resistance index(RI) and pulse index(PI) in essential hypertensive patients. To study the clinical significance of carotid arterial IMT, renal arterial RI and PI in essential hypertensive patients. Methods There were 102 patients in the essential hypertensive group and 22 persons being healthy in the control group. The carotid arterial IMT, renal arterial RI and PI were examined with a high-resolution ultrasound equipped with color Doppler system. Simultaneously their biochemistry indexes,such as blood urea nitrogen(BUN ), serum creatinine (SCr) and so on were examined. The relationship between carotid arterial IMT and renal arterial RI, PI was compared, so did renal arterial RI, PI and BUN, SCr. Results The correlation coefficients ( r ) of the carotid arterial IMT with renal arterial RI and PI were 0.49,0.26 ( P〈0.05) respectively; With the prolongation of the'essential hypertension course, their carotid arterial IMT, the renal arterial RI and PI showed increasing tendency, also had significant difference. The correlation coefficients (r) of the renal arterial RI with BUN and SCr respectively were 0.37,0.20 ( P 〈0.05) ; So did the renal arterial PI with their BUN and SCr( r =0.41,0.20, P〈0.05). The carotid arterial IMT and the renal arterial RI and PI in essential hypertensive group were obviously higher than those of the normal control group, which had the significant difference. Conclusion ①The carotid arterial IMT was positively correlated with the renal RI,PI. ②The increase of the carotid arterial IMT,the renal RI and PI kept company with the increase of the essential hypertension course. ③The renal RI, PI were positively correlated with BUN and SCr. ④The carotid arterial IMT,the renal RI and PI in the hypertension group were higher than those of the normal control group.
