南京市儿童血清25-羟维生素D、甲状旁腺激素和骨特异性碱性磷酸酶水平调查 被引量:26

Study on the levels of 25-hydroxyvitamin D,parathyroid hormone,bone-specific alkaline phosphatase of children aged 0~10 years in Nanjing.

摘要 【目的】了解南京市儿童血清25-羟维生素D[25-hydroxyvitaminD,25-(0H)D]、甲状旁腺激素(parathy-roid hormone,PTH)和骨特异性碱性磷酸酶(bone—specificalk aline phosphatase,BAP)的水平,为临床应用维生素D制剂提供参考。【方法】以儿童保健健康检查门诊、幼儿园及住院共142名0~10岁儿童为研究对象(76名健康儿童,61名上感儿童,5名肺炎儿童),取清晨空腹静脉血,用酶联免疫法检测血清25-(OH)D、PTH和BAP的水平。【结果】南京市健康儿童血清25-(OH)D平均水平为(80.5±29.3)nmol/L,PTH为(1.8±0.9)pmol/L,BAP为(71.0±30.3)ug/L;如以25-(0H)D〈50nmol/L作为维生素D缺乏的标准,则其发生率为10.5%;上述指标随年龄增长呈有规律的波动趋势;患病儿童血清25-(0H)D平均水平为(65.6±32.3)nmol/L,低于正常儿童(P〈0.05)。【结论】南京市健康儿童的维生素D营养状况较好;血清25-(OH)D水平随年龄波动;疾病状态下维生素D水平明显降低。 [ Objective] To get the data of serum 25-hydroxyvitamin D [ 25 (OH) D], parathyroid hormone(PTH), bonespecific alkaline phosphatase(BAP) levels of 0410 y old children in Nanjing city, and to get some potentially useful data for the clinical practice of vitamin D supplements. [Methods] 142 subjects(66 were sick) aged 0-10 y were recruited during 2007/12 -2008/03. Draw venous blood from these subjects after an overnight fast in the morning, and quantitatively determine the serum 25-(OH)D,PTH and BAP levels of the blood samples with ELISA method. [Results] The average level of serum 25-(OH)D was (81.3±29.3) nmol/L in all healthy children, vitamin D deficiency[25-(OH)D〈50 nmolA.] was observed in 10. 5% of these children. The levels of serum 25-(OH)D,PTH,BAP fluctuate with increasing of age regularly. The level of serum 25-(OH)D was (65.6±32.3)nmol/L in sick children,significantly lower than that of healthy children(P〈0.05). [Conclusions] The vitamin D status of Nanjing children is quite well. With increasing of age,the level of serum 25-(OH)D significantly fluctuates, ascending first and descending later; On the contrary, the levels of serum PTH and BAP descend first and ascend later; Sick children have significantly lower level of serum 25-(OH)D than healthy children.
