摘要 目的:从尿液代谢组学的角度研究慢性肾病Ⅲ期肾阳虚证患者代谢物质的变化。方法:选择慢性肾病Ⅲ期患者60例(其中肾阳虚者30例,非肾阳虚者30例)及体检健康志愿者30例,运用代谢组学指纹谱方法进行观察研究。结果:代谢主成分得分值的空间分布表明肾阳虚、非肾阳虚和正常组有非常显著的差异,R2X=0.807>0,充分说明从代谢水平上可以明显观察到慢性肾病Ⅲ期与正常样本的代谢模式差异。在PC1维上的明显分离,R2Y=0.949>0,Q2Y=0.787>0,显示代谢水平上慢性肾病Ⅲ期与正常样本的代谢模式差异非常显著,建立的预测能力也很强。该模型能够很好地区别肾阳虚和非肾阳虚两组病人,R2Y=0.699>0,但是建立的预测能力也很弱,Q2Y=-0.607<0。导致这种预测能力下降可能和肾阳虚和非肾阳虚两组病人都是慢性肾病有关。25个差异代谢物排序后,经过标准品鉴定差异性物质的关键代谢成分分别是丙氨酸、胺基丙二酸二乙酯、脯氨酸、柠檬酸、马尿酸和组胺等物质,并且明确其变化方向和代谢途径,能够很好地区分阳虚与非阳虚的差异性,是分析肾阳虚证的良好手段。结论:代谢组学方法能够很好地区分阳虚与非阳虚的差异性,是分析肾阳虚证的良好手段。 Objective: To observe the urine metabonomic in chronic kidney disease Ⅲ with kidney-yang deficency. Methods: Metabonomics was used to study 60 patients with chronic kidney disease Ⅲ (kidney-yang deficiency 30 and non-kidney yang deficiency 30) and 30 healthy volunteer. Results: Principal components analysis (PCA) found that singulorum sample metabolism principal constituent score spot spatial distribution indicated kidney-yang deficiency,nonkidney-yangdeficiency and normal control had significant difference, R2X=0.807 〉 0.This result hint difference metaboilic level between CKD Ⅲ and normal sample. The finding PCA hint obviously difference in PC1 axis, R2Y=0.949 〉 0,Q2Y=0.787 〉 0, this result not only showed difference metaboilic level between CKD m and normal sample,but also there was very good prognostication capability. Orthogonal signal correction and partial least squares (OPLSDA) to discern that model can distinguish kidney-yang deficiency and non-kidney-yang deficiency, R2Y=0.699 〉 0,but, prognostication capability was very weak,Q2Y = - 0.607〈0.It was a possible reason to explain this descending prognostication capability that these two groups of patients pertain to CKD. Conclusion: There was respective tendency in metabolism principal constituent of every sample spatial distribution score manifest deficiency of kidney-yang, non-deficiency of kidney-yang and healthy group. Metabonomic method can distinguish yang deficiency and nondeficiency-yang with variability, it was a very good method to analyze pattern of deficiency of kidney-yang.
