表皮细胞生长因子复合地塞米松及含奥硝唑的壳聚糖膜修复兔口腔溃疡 被引量:5

摘要 背景:以往采用散剂、软膏、糊剂类型的药物修复口腔溃疡,药物作用时间短,易受到唾液的影响,局部有效药物浓度低。目的:制备表皮细胞生长因子复合地塞米松及含奥硝唑的壳聚糖复合药膜,观察其对口腔溃疡愈合过程的影响。设计、时间及地点:随机分组设计、动物对照观察实验,于2002-09/2003-07在青岛大学医学院附属医院口腔实验中心完成。材料:新西兰兔60只用于制备口腔溃疡模型。壳聚糖、聚乙烯醇、海藻酸钠、羧甲基壳聚糖、表皮细胞生长因子。方法:以壳聚糖、聚乙烯醇、海藻酸钠、羧甲基壳聚糖为成膜材料,加入表皮生长因子、地塞米松、奥硝唑等不同成分制备口腔溃疡复合药膜。采用99%乙酸烧灼法制备兔口腔溃疡模型。60只兔按随机数字表法分为5组:对照组、奥硝唑组(含有奥硝唑的壳聚糖膜)、地塞米松组(地塞米松+含有奥硝唑的壳聚糖膜)、表皮细胞生长因子组(表皮细胞生长因子+含有奥硝唑的壳聚糖膜)、复合膜组(表皮细胞生长因子+地塞米松+含有奥硝唑的壳聚糖膜),每组12只。主要观察指标:每组各取4只进行大体观察,测量溃疡面积,计算溃疡面的体积及溃疡愈合速度,肉眼观察溃疡面水肿、充血程度,愈合情况。其余8只在2,4,6,8d麻醉后各处死2只,行病理组织学观察,记录成纤维细胞数目、炎症细胞数目。结果:动物实验显示,在不同观察时间复合膜组溃疡面积均小于对照组(P<0.05);复合药膜组口腔溃疡平均愈合速度快于对照组(P<0.05);复合药膜组组织学检查表明炎症细胞数目少于对照组(P<0.05),成纤维细胞数目高于对照组(P<0.05)。结论:表皮细胞生长因子复合地塞米松及含奥硝唑的壳聚糖复合药膜可显著加快兔口腔溃疡的愈合速度,促进组织的修复。 BACKGROUND: Previously, dental ulcer is treated with powder, ointment and paste, However, they hold a short drug action and are easily affected by saliva, thus drug concentration reduces in local site. OBJECTIVE: To explore the preparation of epidermal growth factor, dexamethasone and ornidazole-contained chitosan composite membrane, and to test its effect on dental ulcer repair. DESIGN, TIME AND SETTING: Randomized control observation was carried out in the Stomatological Experimental Center of Affiliated Hospital of Qingdao University Medical College from September 2002 to July 2003. MATERIALS: Sixty New Zealand rabbits were used to prepare the dental ulcer models. Chitosan, polyvinyl alcohol, sodium polymannuronate, carboxymethyl chitosan and epidermal growth factor were used in this study. METHODS: Using chitosan, polyvinyl alcohol, sodium polymannuronate and carboxymethyl chitosan as carriers, composite chitosan membrane was prepared by adding epidermal growth factor, dexamethasone and ornidazole as addition. Rabbit dental ulcer was induced by 99% acetic acid. Sixty rabbit models were divided into five groups: control group, ornidazole group (ornidazole-contained chitosan membrane), dexamethasone group (dexamethasone + ornidazole-contained chitosan membrane), epidermal growth factor group (epidermal growth factor + ornidazole-contained chitosan membrane) and composite membrane group (epidermal growth factor + dexamethasone + ornidazole-contained chitosan membrane). Each group contained 12 rabbits. MAIN OUTCOME MEASURES: Four animals from each group were adopted for the gross observations, including ulcer area, ulcer volume and healing speed. Macroscopic observation consisted of edema, hyperemia and healing of ulcer surface. The other 8 animals of each group were sacrificed ar days 2, 4, 6 and 8 under anesthesia, with 2 ones at one time point. They were processed into pathohistological examination, the amounts of fibroblasts and inflammatory cells were recorded. RESULTS: In rabbit dental ulcer experiments, the size of dental ulcer treated with composite membrane was signifcantly smaller than those in control group (P 〈 0.05). The average healing time of ulcer treated with composite membrane was shorter than control group (P 〈 0.05). The number of inflammatory cells was less and the number of fibroblast cells was more in ulcer area in composite membrane group compared with control group (P 〈 0.05). CONCLUSION: The epidermal growth factor, dexamethasone and ornidazole-contained composite chitosan membrane could help to accelerate the healing of dental ulcer and promote the recovery of tissues.
