摘要 目的探讨Nd∶YAP激光豚鼠鼓膜造孔的安全性。方法豚鼠54只,以激光照射时间分为0.5s组、1.0s组和1.5s组,每组16只,另设对照组6只;每个实验组以激光功率分为8个亚组。在耳内镜引导下将Nd∶YAP激光光纤(直径0.5mm)对准鼓膜前下象限非接触照射造孔,在光镜下观察激光对中耳及内耳的影响。结果照射时间0.5、1.0、1.5s,功率4.2-4.6、3.9-4.3和3.2-3.6W的Nd∶YAP激光行豚鼠鼓膜造孔,孔径0.5-1.2mm,中耳及内耳均无明显损伤。结论以适当的照射时间和功率行Nd∶YAP激光鼓膜造孔安全有效,可避免中耳及内耳损伤。 Objective To explore the optimal power and working time of Nd : YAP laser for the tympanostomic treatment in guinea pig models. Methods Fifty four guinea pigs as the experimental models were divided into four groups : 0. 5 s , 1.0 s and 1.5 s expeimental groups according to the working time duration of laser, with 16 guinea pigs in each group, and a control group with 6 guinea pigs. There were eight subgroups (two guinea pigs in one subgroup) involved in every experimental group by difference of working power of the laser. Nocontacted Nd:YAP laser tympanostomy (the diameter of the laser optical fiber was 0. 5 mm) in anterio-inferior quadrant of the membrane was conducted assisted by otic endoscope in the ears of guinea pigs with different power of Nd: YAP laser variant with the working time. The damages by laser irradiations in middle and inner ears were evaluated under the light microscope. Results Tympanostomy was conducted by Nd : YAP laser in the ears of guinea pigs with the power of 4. 2-4, 6, 3.9-4. 3 and 3.2-3.6 W with the working time variously at 0.5 s, 1.0 s and 1.5 s,respectively and with the diameter at 0. 5-1.2 mm. There were no obvious damages could be seen. Conclusion In guinea pig models,it is safety and no obvious damages could be seen in middle and inner ears when tympanostomy conducted by Nd : YAP laser with the optimal power and working time.
