摘要
目的探讨原发性高血压左心室不同构型QT离散度(QTd)的特点。方法对120例原发性高血压患者均进行心脏彩色超声心动图和常规12导联心电图(纸速50 mm/s)检查,算出左心室重量指数(LVMI)和相对壁厚度(RWT),按2个指标来划分左心室构型,同时计算出每位患者的QTd值,比较不同构型组QTd的差异(用F和q检验)。结果得出Ⅰ型正常左室构型30例,Ⅱ型向心性重构40例,Ⅲ型向心性肥厚35例,Ⅳ型离心型肥厚15例;离心性肥厚组的QTd值长于其它各组,向心性肥厚组的QTd值长于向心性重构和正常构型组,而向心性重构组的QTd数值虽稍大于正常构型但无统计学上的差异。结论原发性高血压左心室舒张功能异常发生多先于收缩功能,左心室构型变化越大,QTd也越大。
Objective To investigate the distinction of QT dispersion of different left ventricular geometric patterns in essential hypertension. Methods One hundreds and twenty patients with essential hypertension underwent color echocardiogram and routine 12 lead ECG checking (speed 50 mm/s) ,then the LVMI and RWT were calculated according to the results of above the echocardiogram checking. And each patient's QTd was calculated too. Left ventricular geometric patterns were identified according to the calculating's result and then the QTd was compared among different left ventricular geometric patterns applying the method of T and q testing. Results Displaying pattern Ⅰ (normal geometric pattern group) was found in 30 cases,pattern Ⅱ(remodeling to the heart) in 40 cases, pattern Ⅲ(hypertrophy to the heart)in 35 cases,and patternⅣ (hypertrophy from the heart) in 15 cases, the pattern IV ' s QTd was the longest among the four patterns, the pattern Ⅲ 's QTd was longer than patterns Ⅱ and Ⅰ , the pattern Ⅱ's QTd was no different from pattern Ⅰ . Conclusion The abnormality of left ventricular diastplic function is prior to the systolic function,the bigger left ventricular pattern's extent is,the longer QTd is.
