阻塞性睡眠呼吸暂停低通气综合症与脂蛋白(a)及心肌肌钙蛋白水平变化的关系 被引量:2

摘要 目的:研究阻塞性睡眠呼吸暂停低通气综合症(OSAHS)患者血中脂蛋白(a)[Lp(a)]和心肌肌钙蛋白(cTnI)的水平变化,探讨其临床意义。方法:将79例OSAHS患者按呼吸暂停低通气指数(AHI)、最低血氧饱和度(SaO2 min)分为轻度组、中度组和重度组,另取同期40例健康体检者作为对照组,分别测定其Lp(a)、心肌cTnI水平进行比较。结果:OSAHS中、重度组患者血清Lp(a)水平分别为26.3±9.5 mg/L、29.5±10.6 mg/L,显著高于对照组和轻度患者,差异有统计学意义(P<0.01);心肌cTnI水平分别为6.1±1.9μg/L、6.7±2.3μg/L,显著高于对照组和轻度患者,差异有统计学意义(P<0.01);OSAHS中、重度患者治疗后AHI及Lp(a)及心肌cTnI水平均显著低于治疗前,差异有统计学意义(P<0.05或P<0.01)。结论:OSAHS患者可出现脂质代谢紊乱,出现心肌受损,导致心血管疾病的发生;早期发现和治疗是有效的方法。 Objective: To study the plasma level change of lipoprotein (a) [ Lp (a) ] and cardiac troponin Ⅰ(cTnⅠ) of patients with obstructive sleep apnea and hypoventilation syndrome (OSAHS) and explore the clinical significance. Methods:The 79 patients with OSAHS were divided into three groups ( mild group, moderate group and severe group) according to apnea and hypoventilation index (AHI) and minimal saturation of blood oxygen (SaO2 min ). 40 healthy subjects were recruited as control group. The plasma level of Lp(a) and cTnⅠ were determined and compared. Results :The plasma level of Lp(a) of moderate and severe group were (26. 3 ±9. 5) mg/L and (29.5 ± 10. 6) mg/L which were much higher than that of control group and mild group with statistic significance (P 〈0. 01 ). The plasma level of cTnⅠ of moderate and severe group were (6. 1 ± 1.9) μg/L and (6. 7 ± 2. 3 ) μg/L which were much higher than that of control group and mild group with statistic significance (P 〈 0. 01 ). The AHI, plasma level of Lp(a) and cTnⅠ of moderate and severe group were much lower after treatment than that before treatment with statistic significance ( P 〈 0.05 or P 〈 0. 01 ). Conclusion : Patients with OSAHS may develop lipid metabolic disorder and cardiac injury which lead to cardiovascular disease. It is a good way to be discovered and treated early.
