摘要
目的分析再次穿透性角膜移植术的病因。方法对再次穿透性角膜移植术的94例（95眼）患者的原发疾病和再次手术的直接原因进行回顾性研究。结果本组病例中，男性69例（70眼），女性25例（25眼），年龄2～80岁，平均（43±2）岁。所有病例的原发病因依次为：感染性角膜炎36例（36眼，37．9％），角膜白斑23例（23眼，24．2％），大泡性角膜病变12例（12眼，12．6％），角膜烧伤10例（11眼，11．6％），角膜营养不良或变性5例（5眼，5．3％），圆锥角膜1例（1眼，1．1％），其他7例（7眼，7．4％）。再次角膜移植术的直接原因依次为：移植物排斥81例（83眼，87．4％），慢性角膜内皮失代偿6例（6眼，6．3％），原发性角膜内皮衰竭3例（3眼，3．2％），原发病复发3例（3眼，3．2％）。结论再次角膜移植术的直接原因以移植物排斥为主，原发病因以感染性角膜炎为主。
Objective To examine the major causes of repeat penetrating keratoplasties （PKPs）. Methods Ninty-five eyes of 94 patients who received PKP were reviewed retrospectively for the primary diseases for their first PKPs and the direct causes of repeat PKPs. Results The subjects included 69 men（70 eyes） and 25 women（25 eyes） , aging from 2 to 80 years, with an average age of （43 ±2） years. The causes of the first PKPs, in order of decreasing frequency,were infective keratitis（36 cases,36 eyes ,37.9% ）, corneal leukoma（23 cases,23 eyes,24.2% ）,bullous keratopathy （12 cases, 12 eyes, 12.6% ）, corneal burns （10 cases, 10 eyes, 11.6% ）, corneal dystrophy and degeneration（ 5 cases ,5 eyes,5.3% ）, keratoconus （ 1 cases, 1 eyes, 1.1% ） and others （7 cases,7 eyes,7.4% ）. The direct causes of repeat PKPs, in order of decreasing frequency were graft rejection（ 81 cases ,83 eyes ,87.4% ）, endothelial decompensation（6 cases ,6 eyes,6.3% ）,primarily endothelial failure（3 cases ,3 eyes ,3.2% ） and recurrence of primary diseases （3 cases,3 eyes, 3.2% ）. Conclusions The leading cause of repeat PKPs was graft rejection, and the major primary disease of repeat PKPs was infective keratitis.
