摘要
目的:探讨胰岛素样生长因子-1(insulin-like growth factor1,IGF-1)对乳腺癌的诊断价值,寻找新的乳腺癌早期诊断标志物。方法:对45例经病理证实的乳腺癌及20例乳腺良性病变患者的术前外周血标本进行研究,以20例健康女性外周血标本作对照,应用免疫放射分析方法,定量检测IGF-1在血清中的含量。结果:对照组、乳腺良性病变组、乳腺癌组血清IGF-1值分别为136.50(63.07~190.23)ng/ml、60.02(20.46~151.20)ng/ml、165.50(76.89~584.18)ng/ml,乳腺癌组明显高于对照组(P<0.01),对照组与乳腺良性病变组无显著性差异(P>0.05)。乳腺癌组临床分期Ⅰ~Ⅱ期和Ⅲ~Ⅳ期患者血清IGF-1值分别为156.62(76.89~248.20)ng/ml、250.18(135.78~584.18)ng/ml;有、无淋巴结转移患者IGF-1值分别为224.87(103.28~584.18)ng/ml、139.78(76.89~243.68)ng/ml;IGF-1水平与乳腺癌的淋巴结转移和临床分期密切相关(P<0.01)。在乳腺癌组中雌激素受体阳性、阴性患者中血清IGF-1值分别为239.78(135.78~533.21)ng/ml、154.37(76.89~584.18)ng/ml,IGF-1水平在雌激素受体阳性组与阴性组有显著性差异(P<0.05)。结论:IGF-1的定量检测可提高对乳腺癌的诊断水平,IGF-1有望成为乳腺癌诊断的血清标志物。
Objective: To investigate the value of quantitative detection of insulin-like growth factor-1 (IGF-1) in the diagnosis of breast cancer. Methods: The serum IGF-1 level in 45 cases of breast cancer, 20 cases of benign breast disease and 20 healthy controls was determined by immunoradiometric assay. Results: IGF-1 levels were significantly higher in breast cancer cases than in the healthy controls and the cases of benign breast disease (P〈0.01). No significant difference was found in the serum level of IGF-1 between the patients with benign breast disease and the healthy controls (P〉0.05). All samples were classified as stage I-Ⅱ or stage Ⅲ-Ⅳ, without or with lymphatic metastasis, and estrogen receptor nega-tive or positive. In stage Ⅲ-Ⅳ cases, cases with lymphatic metastasis, and cases positive for estrogen receptor, the levels of IGF-1 were higher (P〈0.01). A significant difference was found in the serum IGF-1 levels between the cases with posi- tive estrogen receptor and those with negative estrogen receptor(P〈0.05). Conclusion: Quantitative determination of IGF-1 is valuable and may be used for early diagnosis of breast cancer.
