正常儿童心脏功能组织多普勒特点及临床意义 被引量:2

Assessment of myocardial function and its clinical value in healthy children using Doppler tissue imaging

摘要 目的应用组织多普勒成像(DTI)检测不同年龄正常儿童的心脏节段运动,评价其心脏功能随年龄变化的特点。方法应用DTI检测228例正常儿童的18节段心肌运动的收缩期峰值速度(Vsm)、舒张早期峰值速度(Ve)、舒张晚期峰值速度(Va)、Ve/Va、达峰时间指数(Tqs/Tc)、射血时间指数(Tqe/Tc)、等容舒张时间指数(Tid/Tc)、等容收缩时间指数(Tis/Tc)及二尖瓣口血流频谱E/A。结果各年龄组心脏DTI参数平均测值之间存在差异,左室各节段平均Vsm、Ve及Ve/Va随年龄增大而增加,与左室舒张末内径(LVEDD)、年龄、心动周期(Tc)等呈正相关(P<0.05);Va则逐渐降低,与年龄、LVEDD及Tc呈负相关(P<0.05);Tis/Tc、Tqe/Tc、Tqs/Tc与年龄呈负相关(P<0.05);各年龄组的二尖瓣口血流频谱E/A与二尖瓣环Ve/Va均呈正相关(P<0.05)。结论正常儿童心脏DTI参数受年龄等因素的影响,因此评价儿童心脏功能时,应考虑年龄因素对评估值的影响。 Objective To determine the normal range of Doppler tissue imaging (DTI) indexes of left ventrical regional wall motion in healthy children, and to assess the influence of age on myocardial function. Methods Two hundred and twenty eight consecutive children aged from 0 to 15 years were enrolled in the study. DTI parameters of 18 segmental wall motion, including peak systolic velocity (Vsm), peak early diastolic velocity(Ve), peak late diastolic velocity(Va), Ve/Va, time to peak systolic velocity index(Tqs/ Tc), quick ejection time index( Tqe/Tc), isovolumic systolic time index(Tid/Tc), isovolumic diastolic time index(Tis/Tc) and mitral flow E/A ratio were assessed. Results Some DTI indexes in systolic and early - diastolic period (Vsm, Ve,Ve/Va) increased with advancing age, left ventricular end-diastolic dimension(LVEDD),Time of cardiac cycle (Tc), and age showed positive correlations ( P 〈0.05) with them. Whereas DTI index in late diastolic period(Va) and some DTI time indexes (Tis/Tc, Tqe/Tc, Tqs/Tc)decreased gradually, and showed negative correlations( P 〈 0.05)with age, LVEDD and Tc, respectively. There was significantly positive correlation ( r = 0.41, P 〈 0.05) between mitral inflow E/A ratio and the corresponding DTI Ve/Va ratio at the mitral annuli. Conclusion This study demonstrates the important clinical effects of cardiac growth and age on DTI indexes in neonates and children. It is necessary to consider the effects of age when evaluating myocardial function of children.
