摘要
目的:探讨趋化因子受体CCR7在不同类型肺癌组织中的表达及其临床意义.方法:采用免疫组织化学方法检测110(鳞癌40,腺癌40,小细胞癌30)例肺癌和30例癌旁正常肺组织中CCR7的表达.结果:在110例肺癌组织中有77例CCR7表达阳性(70.0%),30例癌旁正常肺组织中仅5例表达阳性(16.7%,P<0.05).鳞癌、腺癌及小细胞癌CCR7阳性率分别为60.0%,92.5%,53.3%(P<0.01);腺癌CCR7阳性率明显高于鳞癌和小细胞癌(分别P<0.01,P<0.01).68例伴淋巴结转移的病例CCR7阳性率83.3%,42例不伴淋巴结转移的病例CCR7阳性率47.6%(P<0.01).Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期肺癌病例CCR7阳性率分别为59.5%,76.2%,69.2%,100.0%(P<0.05).不同性别患者肺癌组织中CCR7阳性率分别为男66.7%,女80.8%(P>0.05).小于等于60岁和大于60岁年龄组肺癌组织中CCR7阳性率分别为70.3%,69.6%(P>0.05).结论:CCR7上调表达是肺鳞癌、腺癌及小细胞癌的一个普遍特征,高水平的CCR7表达与肺癌的淋巴结转移和高TNM分期有关,而与年龄、性别无关.
AIM: To investigate the expression of CC chemokine receptor 7 (CCR7) in different types of lung carcinoma and its clinical significance. METHODS: The expression of CCR7 was tested by imunohistochemical method in 110 cases of lung carcinoma specimens (40 cases of squamous cell carcinoma, 40 cases of adenocarcinoma, 30 cases of small cell carcinoma) and 30 cases of pericancerous normal lung specimens. RESULTS: In 110 cases of lung carcinoma specimens, 77 cases showed positive staining of CCR7 protein (70.0%). But in 30 normal lung specimens, there were 5 positive cases (16.7%, P 〈0.05 ). The positivity rate of CCR7 expression in squamous cell caicinoma, adenocarcinoma and small cell carcinoma was 60.0%, 92.5% and 53.3% respectively (P 〈0.01 ). The positivity rate of CCR7 expression in adenocarcinoma was significantly higher than that in squamous cell caicinoma and small cell carcinoma (P 〈0.01 and P〈0.01 respectively). In 68 cases with lymph node metastasis and 42 cases without lymph node metastasis, the positivity rate of CCR7 expression was 83. 3% and 47. 6% respectively (P 〈 0.01 ). The positivity rate of CCR7 expression in stage Ⅰ , Ⅱ, Ⅲand Ⅳwas 59.5%, 76.2%, 69.2% and 100.0% respectively ( P 〈 0. 05 ). The positivity rate of CCR7 expression in males and females was 66.7% and 80.8% respectively (P 〉 0.05 ). The positivity rate of CCR7 expression was 70.3% in ≤ 60 years group and 69.6% in 〉60 years group respectively (P 〉 0.05). CONCLUSION: Up-regnlated expression of CCR7 is a general feature of lung carcinoma, including squamous cell carcinoma, adenocarcinoma and small cell carcinoma. High level expression of CCR7 is associated with lymph node metastasis and higher TNM stage of lung carcinoma, but is independent of age and sex.
