壁冠状动脉的多层螺旋CT表现:与导管法造影的对照研究 被引量:10

摘要 目的评价MSCT显示壁冠状动脉的价值和限度。方法108例临床诊断或可疑心肌缺血的患者做MSCT冠状动脉成像检查,其结果与导管法冠状动脉造影对照。结果本组108例病例中,30例患者CT和/或造影检出壁冠状动脉。MSCT显示壁冠状动脉的敏感度和特异度分别为95.7%和91.8%;假阳性率和假阴性率分别为8.2%和4.3%;阳性预测值和阴性预测值分别为75.9%和98.7%;符合率为92.6%。结论在冠状动脉CT图像能够满足管腔评价情况下,MSCT显示壁冠状动脉的准确性很高。在壁冠状动脉的临床诊断方面,MSCT可以部分取代传统的导管法冠状动脉造影。 Objective To evaluate the accuracy of multi-slice spiral CT in detecting intramural coronary arteries. Methods A total of 108 patients with known or suspected coronary artery ischemia were studied by multi-slice spiral CT. The results were compared with catheterized coronary angiography. Results In the 108 patients, 30 patients with intramural coronary artery were detected by CT and / or coronary angiography. These values correspond to a sensitivity of 95.7%, specificity of 91.8%, positive predictive value of 75.9%, and negative predictive value of 98.7%, coincidence rate of 92.6% for the detection of intramual coronary arteries by multi-slice spiral CT. Conclusions Multi-slice spiral CT permits the visualizaion of intramural coronary arteries with high accuracy if image quality is sufficient.
