原发性肝细胞癌患者免疫功能的改变 被引量:4

摘要 目的:了解原发性肝细胞癌(HCC)患者免疫功能的变化.方法:应用流式细胞术对45例来自不同分期的原发性肝癌患者外周血、肿瘤组织和30例正常人外周血CD4+,CD8+,CD4+/CD8+,CD4+CD25+,CD4+CD25+CTLA4+-T淋巴细胞亚群的比例进行分析比较,以酶联免疫吸附试验(ELISA)检测细胞培养上清中细胞因子白细胞介素-2(IL-2)和转化生长因子β1(TGF-β1)水平.结果:不同分期HCC患者外周血淋巴细胞总数低于正常对照组,CD4+CD25+-Treg及CD4+CD25+CTLA4+-T细胞比例均高于正常对照组,差异有统计学意义.而不同分期原发性肝癌患者外周血中CD4+比例、CD8+比例及CD4/CD8比值与正常对照组比较,差异均无统计学意义.不同分期原发性肝癌患者肿瘤组织中CD4+CD25+CTLA4+-T细胞阳性率高于正常对照组,差异均有统计学意义.与对照组比较,原发性肝癌患者各组IL-2水平均下降;T1~T3期组TGF-β1水平高于对照组,但T4期组TGF-β1水平低于对照组.结论:原发性肝癌患者存在明显的细胞免疫功能紊乱和抗肿瘤免疫功能低下.在肝癌患者外周血及肿瘤组织中CD4+CD25+-Treg所占比例均有明显升高,提示CD4+CD25+-Treg可能是原发性肝癌免疫抑制的一个重要原因.TGF-β1,IL-2分子水平检测有助于肝癌患者诊断和预后判断. AIM: To study the change of immunological functions in patients with primary hepatocellular carcinoma ( HCC ). METHODS: The tumor tissue and peripheral blood samples were collected from 45 patients with every stage HCC; meanwhile, peripheral blood samples were taken from 30 healthy people. The populations of CD4^+, CD8^+, CD4^+/CD8^+, CD4^+ CD25^+, CD4^+ CD25^+ CTLA4^+ T subsets were evaluated by flow eytometry and the levels of IL-2 and TGF-β1 were determined by ELISA. RESULTS: The total numbers of lymphoeytes in peripheral bloods samples from different stages of HCC were lower than those of the healthy controls. The percentages of CD4^+ CD25 ^+ -Treg and CD4^+ CD25^+ CTLA4^+ -T eells were higher than those of the healthy controls. These differences were of statistical significance. But, compared with healthy controls, the differences of the proportions of CD4^+ T cell and the CD4/CD8 ratios at different sta- ges of HCC were not statistically significant. Compared with healthy controls , the level of IL-2 in HCC groups decreased, the level of TGF-β1 in T1-T3 stage HCC groups inereased, and the one in T4 stage HCC group deereased. CONCLUSION: HCC patients have a cellular immunity dysfunction and a poor anti- tumor immune response. The percentage of CD4^+ CD25^+ -Treg in peripheral blood and tumor tissue is significantly higher than that in peripheral blood of the healthy eontrols, whieh may be one of the important reasons of immunosuppression in HCC. The deter-mination of TGF-β1 and IL-2 levels are helpful to diagnose HCC and predict the prognosis of HCC patients.
