摘要
目的探讨颅内海绵状血管瘤的病灶特点与手术方式选择的关系。方法回顾性分析1999年1月~2007年1月间手术治疗的52例颅内海绵状血管瘤患者的临床资料、影像资料,按手术方式分成常规手术组、定向组、导航组,分析病灶部位、大小、病灶至皮层距离与手术方式选择的关系。结果52例中,常规手术手术21例,开放定向手术16例,神经导航手术15例。全切48例,预后不良2例,死亡1例。病灶大小均值从常规手术组的31.7±8.7mm,到导航组的24.0±2.6mm和定向组的16.3±3.7mm而递减,三组病灶大小均值差异比较有统计学意义(P<0.01),定向组病灶最小。病灶至脑皮层距离从常规手术组的15.2±4.1mm,到定向组的20.7±3.8mm和导航组的32.8±6.2mm而递增,三组距离均值差异比较有统计学意义(P<0.01),导航组病灶至皮层距离最大。结论采用开放定向或者神经导航手术,可以切除脑部更小、更深在的海绵状血管瘤。
Objective The purpose of this study was to approach the relationship between the focus feature of intracranial cavernoma(ICCA) with surgical methods. Methods 52 cases of ICCA clinical datas and image materials from 1999/1 to 2007/1 were analysised retrospectively, according to surgical methods, the cases were divided into conventional operation group, stereotatic surgery group and neuronavigation group by surgical methods. The relationship between focus site,focus size and distance between focus to cortices with surgical methods selection were analysised. Results Conventional operation was 21 cases,Stereotatic surgery was 16 cases,neuro navigation surgery was 15 cases. Total resection was 48 cases,bad prognosis was 2 cases, 1 case was death. The focus size was descend from conventional operation group 31.7±8.7mm, to neuronavigation surgery group 24.0±2.6 mm until stereotatic group 16.3±3.7mm , the difference between there groups had statistics sense(P〈0. 01), focus size of stereotatic group was most least. The distance between focus to cortices was ascended from conventional operation group 15.2±4. lmm, to stereotatic group 20.7±3.8mm till neuronavigation group 32.8±6.2 mm ,the diversity between three groups had statistics sense(P〈0.01), the distance of navigation group was most maxima. Conclusion Adopted stereotatic surgery or neuronavigation surgery,location of more smaller ,more deeper of ICCA could be excised .
