摘要 目的:观察丙泊酚+局麻涡轮钻拔牙法在困难牙拔除中的应用。方法:选择拔牙患者60例,随机分为治疗Ⅰ组(丙泊酚+局麻涡轮钻拔牙法)、治疗Ⅱ组(局麻涡轮钻拔牙法),对照组(传统拔牙法),记录各组患者麻醉前、局麻时、拔牙中平均动脉压(MAP)、心率(HR)、氧饱和度(SpO2),拔牙时间,拔牙感受。结果:Ⅰ组患者MAP、HR波动小,感觉痛苦小,Ⅰ、Ⅱ组患者拔牙时间短,3组患者SpO2无差异。结论:丙泊酚+局麻涡轮钻拔牙法,值得在困难牙拔除中应用。 Objective: To observe the clinical use of turbine drill under local anesthesia combined with propofol in complicated exodontias. Methods: Sixty patients of complicated exodontias were randomly divided into three groups: Ⅰ group ( turbine drill exodontia under local anesthesia combined with propefol), Ⅱ group ( turbine drill exodontia under local anesthesia) and control group( traditional hammer and chisel exodontia under local anesthesia). Mean artery pressure ( MAP), heart rate (HR) and oxygen saturation (SpO2 ) before anesthesia, during local anesthesia and operation, the time of operation, subjective feel to exodontia were recorded. Results: MAP and HR were kept steadily in Ⅰ group, but fluctuated greatly in Ⅱ and control group during local anesthesia and operation. Patients in Ⅰ group feel relaxation, but patients in Ⅱ and control group feel fear and tension during exodontia. The time of operation was shorter in Ⅰ and Ⅱ group than control group. There was no significant difference among three groups in SpO2. Conclusion: It was valuable to use turbine drill under local anesthesia combined with propofol in complicated exodontias.
