微螺钉种植辅助定位装置的设计及临床应用 被引量:4

Surgical guide for optimal positioning of mini-screw and its clinical evaluation

摘要 目的介绍一种三维种植钉辅助植入装置,评价其对种植钉植入位置准确性的影响。方法临床筛选20例(共40枚)上颌需要强支抗的患者,应用自制夹板作为定位导航装置辅助微螺钉种植支抗的植入,在上颌第二前磨牙与第一磨牙之间植入种植钉,植入后进行微螺钉种植支抗的定位测量。以15例(共30枚)未使用定位辅助装置植入上颌种植钉的患者作对照。结果实验组40枚种植钉在口内植入时间平均10.3个月,其中有2枚脱落,成功率95%。对照组30枚种植钉在口内植入时间平均9个月,其中9枚脱落,成功率70%。微螺钉种植支抗的定位测量结果表明,实验组a点距角分线的垂直距离平均0.05mm;b点距角分线的垂直距离平均0.91mm;c点距角分线的垂直距离平均0.32mm,种植钉距中轴角分线没有出现显著性偏差。对照组没有定位辅助装置种植钉植入方向明显出现了偏差。结论三维种植钉辅助植入装置可以帮助初次使用种植支抗的正畸医师将助攻种植钉准确地植入目标区域。 Objective To introduce a kind of surgical guide splint for optimal positioning of mini-screw used as anchorage in orthodontic treatment and to evaluate the effect of this surgical guide. Methods Twenty cases of maxillary critical anchorage requisition were selected and 40 mini-screws were inserted into the target region in the maxilla by aid of surgical guide splint. After operation, radiography measurement was performed to evaluate the positioning effect of the surgical guide splint. Fifteen cases (30 mini-screws) undergoing similar operation but without the aid of surgical guide served as control. Results In 40 mini-screws inserted with the aid of surgical guide splint, the average survival time of mini-screw as anchorage was 10. 3 months, 2 were lost. The success rate was 95%. The a, b, c point marked on the apical film showed that the vertical distances between those three marked points and the standard line was 0.05 mm 0.91mm, 0.32 mm respectively. There was no significant deviation of mini-srew from the midline between roots in the target region. On the contrary, significant deviation was found in the control group without surgical guide. Conclusion By aid of surgical guide splint, mini-screw could be inserted correctly into the target region.
