摘要
目的探讨准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)治疗近视眼术后非接触眼压测量值(intraocular pressure,IOP)的变化及其相关因素。方法 393例(760眼)施行 LASIK 术的近视患者手术前、后3个月的 IOP 的差值(the underestimation of IOP,AP)与所矫正的等效球镜度数(spherical equiva-lent dioptre,D),术前中央角膜厚度(central corneal thickness,CCT),术前 IOP,术前角膜前表面曲率(cornealcurvature,K),切削深度(ablation depth,T),术前和术后3个月的角膜曲率差(change of corneal curvature,△K),年龄和性别等因素进行相关分析和多元逐步回归分析。结果△P与 D(r=0.380,P=0.000),术前 CCT(r=0.224,P=0.000),IOP(r=0.657,P=0.000),K(r=0.126,P=0.001),T(r=0.389,P=0.000),△K(r=0.363,P=0.000)均有显著相关性。术前 IOP 是影响△P最重要的参数。多元逐步回归分析得出回归方程:AP=~7.722+0.029 T+0.582 IOP-0.005 CCT+0.116 K。结论术前 IOP 是影响 LASIK 治疗近视后 IOP 测量偏低的最重要参数,实际 IOP 越高,LASIK 治疗近视术后 IOP 测量偏低的量也越大。(中国眼耳鼻喉科杂志,2007,7:220-221)
Purpose To explore the changes in intraocular pressure (IOP) and affecting parameters after laser in situ keratomileusis (LASIK) for myopia. Methods In this prospective study, 760 eyes of 393 patients who underwent LASIK were included. Spherical equivalent dioptre (D), preoperative central corneal thickness (CCT), preoperative IOP, preoperative corneal curvature(K), ablation depth (T), change of corneal curvature (△K), patient age and sex were analyzed to determine the contribution and correlation to the underestimation of IOP(△P). Results D ( r = 0. 380,P =0. 000), preoperative CCT ( r = 0. 224, P = 0. 000 ), preoperative IOP ( r = 0. 657, P = 0.000 ), K ( r = 0. 126, P = 0. 001 ) ,T( r =0. 389 ,P =0. 000), △K( r =0. 363 ,P =0. 000) were statistically significant for △P. The preoperative IOP level is the most important parameter that affect △P. The regression equation obtained was △P = - 7. 722 + 0. 029 T + 0. 582 IOP - 0. 005 CCT + 0. 116 K. Conclusion The preoperative IOP level is the most important parameter that affect the underestimation of IOP after LASIK for myopia. The higher the preoperative IOP is, the higher the underestimation of IOP is.
