实时三维超声心动图评估右心室容积及质量——与多层螺旋CT、磁共振显像对比研究 被引量:2

Validation of real-time echocardiography for quantifying right ventricular volume and mass: comparison with multi-detector row CT and MRI

摘要 目的探讨实时三维超声心动图(RT-3DE)测量右室容积(RVV)及右室心肌质量(RVM)的可行性及准确性。方法应用RT-3DE、多层螺旋CT(MDCT)、磁共振显像(MRI)3种无创性技术对10只猪心模型的RVV及RVM进行测量,将3种技术的测量结果分别与实际值进行相关性分析。结果①3种技术所显示的右室心腔影像学形态均与猪心模型右室铸型形态基本一致。②3种技术所测RVV与猪心右室铸型实测体积对照,其差异均无统计学意义(P〉0.05),且呈高度正相关(rMDCT=0.924,rMRI=0.928,rRT-3DE=0.882;P≤0.001)。③3种技术所测RVM与猪心右室实际右室质量差异亦无统计学意义(P〉0.05),亦呈良好相关性(rMDCT=0.897,rMRI=0.901,rRT-3DE=0.822;P≤0.004)。结论与MDCT及MRI比较,RT-3DE能快速、无创测量RVV及RVM,在无创性心脏定量诊断方面具有极大的临床应用潜力。 Objective To investigate the feasibility and accuracy of real-time three dimensional echocardiography(RT-3DE) for quantifying right ventricular volume(RVV) and right ventricular mass (RVM). Methods RVV and RVM of 10 excised porcine hearts were measured by RT-3DE, multi-detector row computed tomography(MDCT) and magnetic resonance imaging(MRI) respectively, and the measured results were applied relation analysis with actual values. Results ①The RV imaging shapes obtained by these 3 techniques were nearly consistent with that of RV cast mould of porcine. ②RVV values measured by these 3 techniques had no significant difference with actual one measured by cast mould ( P 〉 0. 05), furthermore,there was good correlation among them ( rMDCT = 0. 924, rMRI = 0. 928, rRT-3DE = 0. 882, P ≤0. 001). ③Likewise, RVM values measured by these 3 techniques had also no significant difference with actual one measured by cast mould( P 〉0.05), and there was also good correlation among them (rMDCT = 0. 897,rMRI= 0. 901, rRT-3DE = 0. 822, P ≤0.004). Conclusions Comparing with MDCT and MRI, RT-3DE could quantify RVV and RVM quickly and non-invasively,and possess great clinical application perspective in non-invasive cardial quantitative diagnosis.
