摘要
目的探讨体检人群血压水平及其与心电图异常检出率的关系。方法对5480名体检人员系统查体,分析不同性别、年龄人群的血压水平及高血压患病率,以及不同血压水平的异常心电图检出率。结果男性收缩压为(130.5±20.5)mmHg,舒张压为(80.2±27.8)mmHg;女性分别为(120.3±18.5)mmHg、(76.5±27.1)mmHg,男性显著高于女性(均P<0.01);血压水平随年龄增长而增高,男性正常高限血压占12.24%,高血压占35.68%,女性分别为7.47%、21.20%,男性高血压患病率显著高于女性(P<0.01);高血压者临床心电图异常如左室肥厚、左室高电压、ST-T改变、各类早搏、束支传导阻滞、房室传导阻滞、陈旧性心梗等的检出率显著高于非高血压者(均P<0.01)。结论不同性别、年龄血压值水平不同,随着血压升高,临床心电图异常检出率明显增高;医务人员应根据个体差异建立分类护理档案实行分级管理,采取针对性的护理干预措施促进分群健康。
Objective To study the relation of blood pressure level and rates of abnormal clinical electrocardiogram（ECG） in healthy testing population. Methods A cross-sectional examination was performed in 5 480 healthy adults. Hyperten-sion prevalence and relationships of blood pressure classification to detectable rate of abnormal clinical ECG were ana-lyzed. Results Blood pressure in both sexes was increased with age. The average levels of systolic blood pressure （SBP） and diastolic pressure （DBP） were （130.5±20. 5） and （120.3±18.5） mmHg,and （80.2±27.8） and （76.5±27.1） mmHg for male and female respectively. Mean blood pressure in male was higher than that in female （P〈0.01 for all）. The proportions of high-normal BP and hypertension in male were 12.24 % and 35.68 % respectively, as compared with 7. 47% and 21.20% in female, the rate of hypertension in male was higher than of the female （P〈0. 01）. With the in crease of blood pressure, detectable rate of abnormal ECG such as left ventricular hypertrophy, high voltage of left ventricle, ST-T changes, bundle branch block, old myocardial infarction and premature ventricular arrhythmia was in-creased remarkably （P〈0.01 for all）. Conclusion There are statically significant differences in abnormal clinical ECG among different blood pressure classifications. The detectable rate of abnormal ECG findings is increasd with the in-crease of blood pressure. It is very efficient to establish classified archive, perform hierarchical management and direc-tional nurse.
