摘要
【目的】观察中药灌肠、外敷、针刺在治疗粘连性肠梗阻中所起的作用。【方法】根据随机数字表按入院先后顺序将64例患者随机分为单纯西医治疗组(A组)、单纯西医+电针治疗组(B组)、单纯西医+中药+电针治疗组(C组)。3组单纯西医治疗方法相同,均给予胃肠减压、抗感染及支持疗法。B组在A组治疗基础上加电针双侧足三里;C组在B组治疗基础上加通腑泻热合剂保留灌肠,同时用双柏散外敷腹部疼痛部位。观察3组腹痛消失时间、肛门排气时间、梗阻解除时间以及胃肠减压引流管引流量等情况。【结果】A组和B组各有3例中转手术、C组有2例中转手术,其余56例均经非手术治愈。3组患者梗阻解除率分别为85.0%、85.7%和91.3%(P>0.05),但C组腹痛消失时间、肛门排便排气时间、气液平面消失时间均较A、B组缩短(P<0.05或P<0.01),而且胃肠减压引流量第2天起明显减少,与A、B组比较,差异有显著性意义(P<0.05或P<0.01)。【结论】中药灌肠、外敷、针刺等综合疗法可缩短肠梗阻解除及康复时间,表明对粘连性肠梗阻有明显的疗效。
[Objective] To observe the therapeutic effect of external application and enema of herbal medicine combined with acupuncture for adhesive intestinal obstruction （AIO）. [Methods] Sixty-four AIO patients were randomized into three groups by random number. Group A received routine western medical treatment including gastrointestinal decompression, anti-inflammation and supporting therapy, group B was treated with electro-acupuncture （EA） on bilateral Zusanli acupoints, and group C with EA, retention enema with Tongfu Xiere mixture and external application of Shuangbai Powder on abdominal painful sites. Pain-disappearing time, anus-bubbling time, obstruction-relieving time and drainage volume from nutrient canal during gastrointestinal decompression were investigated in the three groups. [Results] There were 3 patients inverting to open surgery in group A, 3 in group B and 2 in group C. The other 56 patients did not receive surgery but were cured. Obstruction-relieving rate was 85.0% in group A, 85.7% in group B and91.3% in group C （P 〉 0.05）. Pain-disappearing time, anus-bubbling time, and time for gas-fluid plane disappearing were shortened in group C, and drainage volume from nutrient canal during gastrointestinal decompression obviously reduced on the second day as compared with those in groups A and B （P 〈 0.05 or P 〈 0.01 ）. [ Conclusion] Comprehensive therapy of external application and enema of herbal medicine combined with acupuncture shortens obstruction-relieving time and promotes recovery, indicating that it is effective for AIO.
