摘要
目的明确胰十二指肠切除术后胆瘘发生的危险因素。方法回顾性分析1994年12月至2004年11月间接受胰十二指肠切除术的连续141例病人的病历资料。结果胆瘘的发生率为9.2%。单变量分析结果表明:糖尿病、术中输血量、手术失血量、胆管直径、术后血清白蛋白及血红蛋白水平为有意义的相关因素;经Logistic回归多变量分析确定了2个独立与胆瘘相关的变量,即手术失血量(OR=6.271)及胆管直径(OR=5.940)。胆瘘的预测方程为:P=1/[1+ e^(-(-3.286+1.836手术失血量+1.782胆管直径))]。结论胆瘘的危险因素为手术失血量≥1000 ml及胆管直径<1.5 cm。
Objective To determine the risk factors associated with development of biliary fistula after pancreatoduodenectomy （PD）. Methods The clinical data of 141 patients receiving PD in our hospital from December 1994 to November 2004 were retrospectively analyzed. Results The incidence of biliary fistula was 9.2%. Univariate analysis showed that diabetes, blood transfusion, operation blood loss, diameter of the biliary tract, postoperative serum albumin and hemoglobin levels were significantly associated with biliary fistula after PD. Multivariate analysis using logistic regression identified 2 variables as independent factors associated with the occurrence of biliary fistula, namely, operative blood loss （OR=6. 271） and diameter of the biliary tract （OR= 5. 940）. The predictive equation was P= 1/[1＋ e^-（-3.286＋1.83 amount of operative blood loss＋1.782 diameter of biliary tract）]. Conclusions The independent risk factors for biliary fistula after PD are operative blood loss more than or equal to 1000 ml and the diameter of the biliary duct less than 1.5 cm.
